Umpqua Health is a Coordinated Care Organization (CCO) in Roseburg, Oregon that connects over 30,000 Douglas County OHP members to physical, dental, and behavioral health services and benefits through an integrated network of providers. Umpqua Health is currently seeking a Program Manager, Douglas County System of Care to join our team.
- Full-time remote/community based and interactive position, with travel requirements.

The Program Manager, Douglas County System of Care will provide dynamic leadership in the development, coordination, implementation, and evaluation of the local Douglas County System of Care (SOC) governance structure. The Douglas SOC is a collaborative network of community services, supports, and resources focused on the total health of families and youth (ages 0-25), experiencing multi-system involvement. The SOC functions to improve services and supports for youth and families. This supervisory position ensures the SOC is functioning in accordance with system of care values and principles while meeting contractual and reporting requirements. Maintain oversight the Wraparound review committee to facilitate collaborative stakeholder action, encourage shared decision-making, and monitor accountability to fidelity of practice model.
ESSENTIAL JOB RESPONSIBILITIES
System of Care & Wraparound Requirements:
- Serve as Subject Matter Expert (SME) of both the System of Care and Wraparound models.
- Stay abreast of local, state, and federal issues that could impact a sustained local SOC and Wraparound.
- Ability to identify and implement strategies aimed at improving integration of care for youth and families
- Lead and provide administrative oversight of the System of Care’s multi-level cross-system governance structure
- Accountable for management and oversight of the Wraparound Review Committee and the System of Care practice-level workgroup, advisory, and executive councils.

System of Care & Wraparound Responsibilities:*
- Lead and continuously develop the Douglas SOC to achieve desired outcomes for youth and families.
- Develop long-and-short-term goals and objectives for the growth and improvement of SOC.
- Ensure continued implementation of policies, procedures, and practices consistent with the values and principles of SOC.
- Ensure continued implementation of policies, procedures, and practices consistent Wraparound services.
- Assess opportunities and barriers to achieve a seamless SOC across all systems and among government departments.
- Improve and maintain barrier submission form process, including barrier tracking and quarterly reporting
- Evaluate the implementation of all aspects of the SOC.
- Assist in the preparation of SOC funding including grant oversight, budgeting, and administers funds as assigned in accordance with pertinent contracts and MOU’s.
- Oversee, revise, and update Wraparound policy and procedure.
- Monitoring, tracking, and reporting of Wraparound Review Committee.
Support/Backbone of System of Care Governance:
- Spearhead the recruitment and retention of youth and families with a focus on underrepresented populations.
- Facilitate collaboration among the system of care partners to ensure consistent implementation of these policies and procedures and to ensure continuous consensus building and ownership.
- Review and update SOC committee Charters and other documents annually or as changes are necessary.
- Prepare and disseminate information to SOC members by establishing solid communication loop(s).
- Facilitate and administratively support the monthly SOC meetings: minutes, agenda, scheduling, and others.
- Organize and provide support to the SOC Executive Council.
- Inform appropriate SOC Council members of actual and/or potential challenges and barriers.
Essential Role Requirements:
- Coordinate cross-system communication and planning.
- Use of data and information to guide program decisions.
- Demonstrates professional ethics.
- Prepares and/or oversees the preparation of records and reports as required.
- Knowledge of culturally specific issues, resources and strengths of the populations served and will have knowledge of culturally specific issues, resources, and strengths of the AI/AN populations.
- Demonstrate an on-going understanding and current knowledge of benefits for Medicare and Medicaid populations.
- Comply with organization’s internal policies and procedures, Code of Conduct, Compliance Plan, along with applicable Federal, State, and local regulations.
QUALIFICATIONS
- REQUIRED- Bachelor’s degree in human services or related field
- PREFERRED- Master’s degree in social work or counseling plus two (2) years’ experience in youth and family serving systems
- PREFERRED- Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT), Licensed Clinical Social Worker (LCSW), or Licensed Psychologist
- PREFERRED- Experience in Behavioral Health, utilization management, case management, Wraparound, and/or managed care.
