Public Health Outreach Worker position based at the

THIS IS AN HOURLY WAGE ( PART - TIME ) POSITION (NO BENEFITS ) THAT CAN WORK UP TO 1,500 HOURS BETWEEN JULY 1ST AND JUNE 30TH EACH YEAR .
This position provides navigation services for high-risk pregnant clients and infants/toddlers, in partnership with RN Case Manager. Responsibilities include assessment of client needs and education about relevant maternal and child health topics to clients. The primary focus will be on assuring that clients access needed medical care and social services resources to achieve a healthy pregnancy outcome and appropriate access to medical services for infants/toddlers up to 2 years of age.

This is a
restricted position
that is solely funded by the Maternal Child Health (
MCH
) grant for a one-year period ending June 30th each year. The availability of funding is scheduled for review annually and periodically thereafter. Employment is contingent on the continued availability of grant funds.
Please note: Wage employees are limited to working no more than 29 hours per week on average over the course of 12 months (1,500-hour limit). Wage employees do not have access to state benefits.

Minimum Qualifications
- The successful candidate should be able to demonstrate proficiency using computer software and databases.
- Ability to communicate effectively orally and in writing.
- Experience working with the public.
- Experience working in an office setting.
- Experience using Microsoft Office
Preferred Qualifications
- Maintains CPR, Childbirth Instructor and NIMS certification.
- Demonstrated knowledge of prenatal/maternity care, including common milestones and signs of problems.
- Demonstrated knowledge of infant/child health principles, immunizations, normal growth and development.
- Knowledge of maternal and child health, community resources, case management principles, mentoring process, individual teaching methods and public speaking and care giving practices.
- A valid State Driver’s License is required to operate a state vehicle or if operating a personal vehicle while conducting business on behalf of the agency.
Special Requirements
Employment is contingent upon satisfactory results of a state and federal criminal history background check and the Department of Social Service’s Child Abuse and Neglect Central Registry check, U.S. HHS IG Exclusion List check, employment reference check and E-Verify. Other financial, credit, driving, or other background checks prior to employment may be required for certain positions. Completion of Statement of Economic Interests will be required upon employment.
