Qualified Health Home Specialist

Hiring Bonus of $1,000 available to external candidates

We are searching for a Qualified Health Home Specialist to support clients in reaching their individual goals and assist team members in coordinating care. The Behavioral Health Home program utilizes a multi-disciplinary team to deliver person-centered services designed to support a person in coordinating care and services while reaching his or her health and wellness goals. We engage our clients and their families in moving toward stability, recovery and self-sufficiency.


Hours: Monday-Friday business hours- Hybrid role Mostly working remote

Location: Minneapolis, MN

Hiring Range: $17.50 - $18.11

Required Qualifications:

  • High school diploma or GED
  • Excellent driving record, reliable car, and current auto insurance

AND ONE OF THE FOLLOWING

  • Certification as a Community Health Worker (through Minnesota State Colleges & Universities)
  • Certification as a Peer Recovery Specialist through the Department of Human Services provided by the Mental Health Consumer/Survivor Network
  • Qualify as a Mental Health Rehabilitation Worker
    • Have a high school diploma or equivalent
      • AND ONE OF THE FOLLOWING
        • have an associate of arts degree;
        • have two years of full-time postsecondary education or a total of 15 semester hours or 23 quarter hours in behavioral sciences or related fields;
        • be a registered nurse;
        • have, within the previous ten years, three years of personal life experience with mental illness;
        • have, within the previous ten years, three years of life experience as a primary caregiver to an adult with a mental illness, traumatic brain injury, substance use disorder, or developmental disability; or
        • have, within the previous ten years, 2,000 hours of work experience providing health and human services to individuals.
  • Associates Degree in behavioral science
  • Two years of full-time work experience with people with serious mental illness

Preferred Qualifications:

  • Experience working with people who have serious mental illness, chemical dependency, seizures, epilepsy, and/or other barriers to service

Main Job Duties:

  • Develop and maintain a collaborative relationships with clients, serving as a role model by demonstrating positive, proactive and respectful communications
  • Coaching clients and their supporters toward increased independence and self-reliance, improved health management and healthy lifestyle changes
  • Connecting clients with services and supporting them in accessing those services
  • Documenting accurate and timely case notes for all client contacts
  • Accompanying clients to appointments as appropriate to the person's needs and circumstances
  • Collaborating with a multi-disciplinary mental health team
  • Promoting recovery, stability and self-sufficiency
