Wraparound Services of the Hudson Valley has an opportunity for a dedicated professional to join our team as a QIDP for our IRA programs located in throughout Ulster County. The QIDP ensures that our individuals receive the highest quality care through the development of person-centered treatment plans as an active member of their service team.
We have 70+ year history of providing quality residential and community based services to people with disabilities of all ages and their families throughout the Hudson Valley. We are known for our enhanced staff training, innovative programs, and caring, supportive services.
We are looking for a professional who can partner with residential teams, both internally and from other agencies as well as families to advocate for program individuals and develop positive, professional relationships with individuals receiving services and their families.
Daily responsibilities include: writing service plans, coordinating services, chairing planning meetings and advocating for individuals served. Applicant should have a minimum of a Bachelor's degree in a human services field and one year experience working with people with developmental disabilities. Experience in a residential program preferred.
Job Type: Full-time
Pay: $21.97 per hour
