Alter Health Group is currently seeking a highly organized, diligent, and ethical individual with Behavioral Health and
Primary Mental Health industry knowledge to join our team as a Quality Assurance Auditor. This individual will work
on site at the San Diego Mental Health facility, with the occasional need to participate in on-site activities at the
corporate office with advance notice. Reporting to the Vice President of Compliance and Policy and the Quality
Assurance Supervisor, the Quality Assurance Auditor will take responsibility for ensuring consistent and accurate
completion of chart and medication audits and timely documentation of error corrections in Mental Healthcare settings.
Requirements:Quality Assurance Auditor Job Responsibilities:
- Chart Audits:
Conduct chart audits on a standing schedule, as directed by supervisor.
Utilize the quality standards and deadlines set forth by all licensing and certifying bodies (e.g., DHCS,
CDSS, TJC) and those set by the managerial staff within the Clinical, Medical, Operational, and
Compliance departments to audit client charts for compliance on a standing basis.
Communicate with staff regarding needed self-correction within a timely manner.
Report deficiencies to designated Managerial and Compliance staff in a timely manner.
Ensure that areas of repeated deficiencies and/or staff conducting repeated deficiencies are reported and
documented within the Audit & Compliance Committee Meetings for additional training and corrective
action.
- Medication Audits:
Conduct audits of all Doctor’s Orders, MARs / MORs, Medication Central Storage documentation,
Medication Destruction document, and any other medication-related documentation either in the chart or
on paper on on a standing schedule, as directed by supervisor.
Utilize the quality standards and deadlines set forth by all licensing and certifying bodies (e.g., DHCS,
CDSS, TJC, CARF) and those set by the managerial staff within the Clinical, Medical, Operational, and
Compliance departments to audit medication components of charts for compliance on a standing basis.
Communicate with staff regarding needed self-correction within a timely manner.
Report deficiencies to designated Managerial and Compliance staff in a timely manner.
Complete Medication Incident reports and documentation for all applicable medication errors found.
Ensure that areas of repeated deficiencies and/or staff conducting repeated deficiencies are reported and
documented in a manner designated by a supervisor for additional training and corrective action.
- Medication Protocol Training Support:
If and when requested by a direct supervisor, assist with and/or conduct supplementary training for direct
care staff on medication management protocols when determined necessary based on error findings.
- Facility Audit Assistance:
If and when on-site, although not a primary responsibility, may conduct environmental facilities audits,
when directed, to ensure adherence to operational and environmental standards.
If and when requested by a Direct Supervisor, assist to audit temperature and log binders.
Survey Preparation Audit Assistance:
When appropriate, assist with licensing survey preparation audits including providing assistance to the
Human Resources department to audit personnel files to the standards set forth by all licensing and
certifying bodies (e.g., DHCS, CDSS, TJC).
- Reporting & Correction of Errors:
Communicate with staff regarding needed self-correction within a timely manner.

Confidentiality:
Maintain the utmost level of confidentiality and integrity as it pertains to client charts and PHI as well as
Human Resources information that is contained within the confidential personnel files undergoing audits.
Quality Assurance Auditor Job Qualifications:
Confidentiality: Demonstrated ability to handle sensitive and confidential information.
Integrity & Character: Individual must possess the ability to remove bias and demonstrate unwavering honesty
in all deficiency and correction reporting duties, in alignment with the Core Values of the organization.
Detail Oriented: Individual must possess a level of attention to detail that is second to none as it pertains to chart
and facility review.
Reporting Skills: Individual must possess strong reporting skills with ability to delineate opinions from facts and
remove all bias.
Communication Skills: Individual must possess professional, tactful communication skills to ensure effective
interdepartmental communications with staff in all departments regarding audit findings and corrections.
Adept Learning Ability: Individual must possess the ability to quickly learn compliance standards for both
charts and facilities as they pertain to both managerial standards and applicable healthcare-related licensing bodies
(e.g., Department of Health Care Services, Department of Social Services, The Joint Commission, Commission
on Accreditation of Rehabilitation Facilities).
Time Management Skills: Individual must demonstrate excellent time management skills with proven ability to
prioritize and meet deadlines.
Computer Literacy & Digital Skills: Individual must be proficient in Microsoft Suite, G Suite, and possess
quick learning skills in regard to information technology.
Quality Assurance Auditor Education and Experience:
College level or continuing education degree in a field of medical focus preferred.
Two years of experience in Behavioral Healthcare and/or Primary Mental Healthcare setting preferred.
