Referral Coordinator Job at MCHC Health Centers
MCHC's Dora Street Health Center in Ukiah, CA is currently seeking a regular, full-time Referral Coordinator. All Referral Coordinators are to set up appointments and provide referral information for MCHC patients needing care from specialists. Provide appropriate scheduling for patients referred to Specialty providers. Acts as liaison between MCHC and outside medical facilities to enable optimal patient care.
Primary Duties Include:
- Provides information to patients regarding specialty services provided at MCHC and at external physician offices, according to the care needed.
- Liaisons with outside medical offices to coordinate specialist referrals.
- Maintains appropriate interaction and communication with medical providers.
- Checks financial eligibility for specialty appointments.
- Schedules specialty appointments at MCHC, following protocol for the individual specialty providers.
- Communicates with the patients to advise of appointment date, time, location, and preparation needed. Provides address or map for location of appointment for patients.
- Documents appointment information or pertinent medical information in patients’ chart as indicated.
- Coordinates financial arrangements with Financial Services, as needed.
- Obtains insurance authorization for specialty appointments when indicated.
- Communicates with outside medical offices for pertinent medical information required for specialty appointments prior to the appointments.
- Prepares appropriate medical information (i.e. labs, x-rays’ reports, etc.) for appointment. Information will be sent prior to the visit, as appropriate.
- Confirms all MCHC specialty appointments, upon request.
- Verifies that specialty appointments have been scheduled.
- Verifies that outside appointments made for MCHC patients by the Referrals Dept. were kept. Obtains written consultation and or chart notes from the encounter.

Minimum Qualifications:
- High School Diploma or General Education Degree.
- Two or more years related experience and/or training or equivalent combination of education and experience
- One or more years’ experience in customer relations.
Preferred Qualification:
- One or more years’ experience in healthcare.
