General Statement of Duties

Under the Director of Nursing, the nurse will provide nursing care for consumers, auditing of records (electronic and Paper), and training for staff to address the unique needs of consumers served in Enable’s programs. The nurse will act as a liaison with outside healthcare providers who provide services to Enable consumers and oversee contracts of health-related entities. Will be asked to cover for other nurses as needed.

Essential Job Functions

  • Ability to Commute daily to multiple group home locations within regions.
  • Documents information on each new consumer, as service requirements dictate, that includes personal health, family health and immunization histories, as well as any special health status, specified laboratory data, past and current medications.
  • Under the direction of the DON, identifies, coordinates and monitors the health care of the consumers, especially those with complex medical needs.
  • Maintains confidentiality with storage of records, medications and consumer communications.
  • Enters data into the system that records all medical visits and validates appointments using written and/or electronic documentation.
  • Provides direct nursing care to consumers, providing assistance particularly in the areas that require skilled nursing care.
  • Assists the DON with planning, coordinating trainings, and/or training employees in medical care and procedures needed to provide excellent direct care.
  • Ensures program records are properly maintained and reports are prepared and submitted on time, including but not limited to, time and attendance, electronic case records, and daily transaction records.
  • Works cooperatively with program management to identify health care needs in all service delivery areas.
  • Cooperates with Enable and NJ, DHS, Division of Developmental Disabilities staff during any inspection or investigation and accreditation reviews.
  • Successfully completes and demonstrates proficiency in all areas of required training.
  • Completes other tasks as assigned by supervisor.

Qualifications

  • Must be 21 years of age or older
  • Licensed Practical or Registered Nurse (LPN or RN) licensed in New Jersey
  • Experience working as a nurse
  • Excellent organizational skills
  • Excellent written, oral and communication skills
  • Strong trainer with excellent presentation skills
  • Ability to effectively communicate with the consumers with whom they are working
  • Ability to effectively complete necessary work-related documentation through paper and computer records systems
  • Valid driver’s license and personal automobile

Preferred:

  • Experience working with individuals with disabilities
