Job Responsibilities

This individual works as a part of our Specialized Outpatient Services team serving individuals in group and private homes in Jackson and Hillsdale county to provide nutritionally-related services that are for rehabilitative purposes to improve the individual's overall health and ability to care for nutritional needs. Duties include:


1. Conducting outcome assessments/evaluation of the individual's nutritional status

2. Interviewing individual and caregivers to assess food preferences and dislikes as well as consumer history.

3. Designs and implements nutrition care plans as indicated by the individual's health status

4. Meets with individual periodically to evaluate the quality of meals served, meal preparation, chewing/swallowing difficulties, poor consumption, etc.

5. Monitor's individual's weight trends and adjusts the treatment plan as needed.

6. Provides dietetics education to caregivers or teams of caregivers

7. Counsels, educations, and/or intervenes in health promotion/disease prevention for individuals needing medical nutrition therapy for common conditions (hypertension, obesity, diabetes, and diverticular disease).

8. Instructs/in-services individuals and their caregivers on nutritional needs, food modifications, and food/drug interactions.

9. Coordinates and modifies nutrition care activities among caregivers

10. Conducts nutrition care component of interdisciplinary team meetings to discuss individual's treatment and future planning.

11. Refers individuals to appropriate community services for general health and nutrition needs and to other primary care providers as appropriate.

12. Documentation of assessment and monitoring activities in an Electronic Medical Record


Required Education/Licenses/Certifications

  • Bachelor's degree in Dietetics from an accredited college or university
  • Active registration with the Commission on Dietetic Registration


Required Experience/Skills

  • Experience working with individuals diagnosed with a mental health disorder and/or development disability
  • Experience in community nutrition programs
  • Knowledge of the Manual of Clinical Dietetics
  • Requires basic skill level working with Microsoft Office and office equipment
  • Ability to work in an office and in community settings in Jackson and Hillsdale counties
  • Ability to travel to community locations and other offices


2-3 days per week (16-24 hours)
