JOB DESCRIPTION
Health Link Home Health Agency is seeking a Full-Time/Part-Time Registered Nurse (RN) to provide care at our patient's homes throughout the OAKLAND AREA and surrounding regions. As a Registered Nurse, you will evaluate the patient's overall health. Your goal will be to improve the quality of life for your patients. All Nursing will be done in accordance with each patient's individual plan of care.
For over 15 years, we continue to serve as a trusted and local in-home health care provider with expert knowledge in rehabilitative care, elderly health care, post-acute and surgical care.
We are looking for compassionate clinicians to join our amazing team of healthcare professionals. Our back office is dedicated to supporting our field clinicians, which truly allows them to function remotely and independently.
- Provide services within the scope of Nursing, in accordance with the plan of care, and in coordination with the Interdisciplinary Team
- Follow plan of care; provide treatment; evaluate patient's reaction to treatment and report any changes in his/her condition to the Director of Nursing
- Online Charting in Kinnser in a timely manner on company-provided devices
- Be responsible for following all company policies and procedures
- Perform other duties as assigned
QUALIFICATIONS
- One year of previous nursing experience
- Current RN license to practice in California
- Current CA Driver's License
- Flexible scheduling

Starting from $125,000 annually DOE
