Oswego Industries, Inc. is urgently seeking a Full-Time & Part-Time Registered Nurse to join our Day Habilitation Team. We are an Agency dedicated to improving the lives of those with intellectual and developmental disabilities and the RN position plays an essential part in fulfilling that mission. While overseeing a small nursing team, the RN will assess the health related needs of individuals participating in the Day Habilitation programs. They provide continuing education training to Direct Support Professionals, and provide direct care to individuals based on their individualized nursing plan of care. The RN serves as a member of the Agency's Safety Committee and helps ensure care delivery is compliant with OPWDD rules and regulations.

Position Summary: Working under the general direction of the Director of Services and Manager of Services, the Registered Nurse will provide direct nursing care by assessing, planning, organizing, documenting, administering and evaluating care provided for the people supported by Oswego Industries and The Arc of Oswego County. Care delivery will be done in compliance with the rules and regulations of OPWDD and the rules and regulations of Oswego Industries and The Arc of Oswego County. The Health Safety Coordinator/RN will monitor and perform direct services in health and safety areas, as well as to participate in specific programmatic instruction to participants.
Duties and Responsibilities:
- Assess health related needs of individuals participating in a day habilitation setting, while coordinating with health care/services providers, clinical, residential staff or families and agency staff to meet these needs.
- Assume primary responsibility for program oversight specifically related to the maintenance of a safeand healthy program area.
- Provide care direction as needed to direct care staff based on the individual’s plan of nursing care.
- Ensure that all individuals’ medical records are in compliance with all applicable regulations. Maintainall appropriate chartings.
- Support the interdisciplinary approach to individual care as well as the team process and participates onthe team as directed.
- Follow up/evaluation of internal events and incident reports in health related areas
- Make reasonable judgements regarding necessary treatments related to participant complaint, injuries,accidents, etc. and carry out emergency, First Aid and/or other care as determined.
- Perform assessments on individuals supported by Agency.
- Arrange medical services.
- Maintain all records related to accident/injury/illness for those in the specified program.
- Assist in writing and reviewing Nursing Policies and Procedures as directed.
- Administer medications as required/indicated and maintain all related records for compliance.
•Provide oversight and recertification to AMAP certified staff.•Utilize principles of infection control when caring for supported individuals.
- Continually model appropriate instructional and care techniques.
•Assume primary responsibility for and carry out participant specific health care activities as required i.e.,tube feedings, soaks, lotion rubs, etc.•Provide restorative and rehabilitative services as necessary, i.e., bowel and bladder training, ostomy
care and delivery of oxygen therapy.
- Take and record vital signs.
- Perform related duties as requested, including, but not limited to, specimen collection and injections.
- Assume primary responsibility for the carry over of physical and occupational therapy activities on a regular basis.
- Collaborate with OI/Arc Plan Coordinators, PT/OT/ST, parents/residential staff, and personnel from other agencies as necessary/required.
- Perform on-going surveillance and nursing intervention to rescue chronically ill persons from the development of negative side effects and secondary results of treatment. Upon discovery of an untoward medical issue, the nurse must take immediate action, including notifying his/her supervisor.
- Conduct and/or coordinate health related training programs as required or desired by the program.
- Maintain nursing office/nursing practice in compliance with OSHA guidelines. Maintain respective nursing offices with supplies as needed, including, but not limited to, OTC medications and durable medical equipment.
- Keep abreast of changes in nursing field; augment current job knowledge by participating in conferences, seminars, workshops, etc., conducted both outside and within the agency.
- Attend team, staff, incident/compliance committees, safety committee and other required meetings
SUPERVISORY RESPONSIBILITIES: Provide coordination, supervision and overall support to medical/nursing staff including LPNs and PCAs.
EDUCATION and/or EXPERIENCE:
- BSN degree required
- At least 1 year experience working with developmentally disabled population, 1 year clinical experience and 2 years’ experience in a human service or healthcare agency.
- Supervisory experience is preferred. A clean valid New York State driver license is required.
