Pearl Harbor, Hawaii - Registered Nurse Medical Home Port (MHP) and Behavioral Health Integration Program (BHIP)

Minimum Qualifications:

· Degree: Minimum of an Associates Degree of Nursing.

· Education: Graduate from a college or university accredited by National League for Nursing Accrediting Commission (NLNAC), or The Commission on Collegiate Nursing Education (CCNE).

· Experience: Possess a minimum of 1-year outpatient experience as an ambulatory care nurse within the last 2 years, with experience providing services in psychiatric nursing, behavioral health and/or wellness counseling.

· License Current, full, active, and unrestricted license to practice as a Registered Nurse.

· Hours: Monday through Friday, between the hours of 0600 and 1800

Duties

· Collaborate and integrate with members of the BHIP-MHP team, MHP team, other departments, and other directorates to achieve optimal care for the patient in the BHIP-MHP model. Liaise between health care providers, MHP nursing staff, health benefits advisors, health care finders, discharge planners, ambulatory and inpatient care managers, population health nurses, and clinic management, thereby providing continuity of care and minimizing fragmentation in the delivery of services.

· Track patients with depression and/or post-traumatic stress disorder (PTSD) and develop a system to track patient participation and outcomes. Services may also be offered for certain patients with numerous medical and or psychosocial concerns that may not include depression or PSTD. Services shall be provided to a full spectrum of patient populations from pediatric to adults.

· Assist primary care manager (PCM) in implementing best-practice guidelines for high-frequency conditions such as depression, common chronic disease management, and behavioral health and wellness support, coordinate acute-care management with MHP team; and use empirically validated screening tools.

· Maintain effective communication with staff, patients, families, and other departments. Facilitate communication between the patient and other providers/departments.

· Follow up with patients referred by the PCM or Internal Behavioral Health Consultant (IBHC) and focus on patient progress in PCM treatment adherence.

· As required, participate in PCM or IBHC case staffing and consultations that involve patients actively engaged in or may benefit in care facilitator services.

· Participate in screening processes developed by the IBHC and PCM to improve the recognition of and appropriate interventions for depression and PTSD.

· As required, provide and teach curriculum content for classes that promote education and skill-building to enhance enrollee population psychological and physical health.

· As part of the MHP team, the HCW shall raise awareness about behavioral health issues and care facilitator services in MHP practice area to promote easy referral of patients.

· Inform the Clinic Manager of administrative/clinical problems as they arise.

· Participate in all accreditation and Medical Home Port recognition processes (JC, IG, NCQA, etc.).

· Engage other MHP team members to assist in improving documentation of the episode of care using MHP Clinic-approved templates as the initial template (i.e., Tri-Service Workflow/COMPASS).

· Coordinate schedule and non-availability (i.e., absence, training, etc.) with other BHIP-MHP team members to ensure adequate access to care.

· Participate in MHP team huddles.

· The HCW may receive a phone issued by the MTF for communication between Makalapa and Kaneohe BHCs, the contractor is responsible for replacement costs should the equipment be lost or stolen.
