Crossroads Community’s Health Home Program offers individually focused care, empowering participants to manage and prevent chronic conditions, with a goal of improving overall health and reducing avoidable hospital visits. Our community based program puts focus on the somatic and mental health needs of our clients in the Psychiatric Rehabilitation Program.

We are looking for someone with the appropriate credentials to serve as the Health Home Program Coordinator. This person would serve as the Nurse Care Manager responsible for services related to comprehensive care management, health promotion, transitional care, individual family support, and provides community and social support referrals. We offer our participants enhanced care-coordination services, work with the participants and their primary care providers, specialists, and behavioral Health providers, to assess and assist with creating goals related to the whole health of the individual. Did you know the average reduction in life expectancy in people with Mental Health conditions is between 10-20 years? This means because of preventable physical disorders this population is dying up to 20 years younger! Our Health Home Program approach is to contribute to healthy life expectancy rates.

  • Ensure that Health Home services are guided by the needs of each person served and is respectful and compassionate.
  • Assess consumer needs and facilitate admission to HH services.
  • Participate in the development of the consumers’ care plans and health care goals.
  • Ensure that consumers have access to appropriate medical and behavioral health services.
  • Assess consumers nutritional needs and available resources to have needs met.
  • Develop and maintain working relationships with Primary and Specialty Care Providers, including inpatient facilities, including contacts for admissions and discharges.
  • Track assessments and screenings to ensure completion as required.
  • Develop and deliver prevention and wellness initiatives and health education for staff and consumers.
  • Consult with PRP and RRP staff regarding consumer health conditions.
  • Maintain accurate and timely documentation and reports.
  • Monitor HIT tools and reports for medication adherence and hospital encounters.
  • Monitor and report performance measures and outcomes and lead improvement efforts.
  • Operate the program within the given financial guidelines.
  • Schedule, supervise and evaluate assigned staff in accordance with policy, ensuring staff accountability for job performance.
  • Participate on CCI’s Leadership Team.
  • Ensure that the Health Home Program operates in accordance with agency policies and legal, regulatory and accreditation standards.
  • Ensure that Health Home Care Coordination is provided to all consenting participants.
  • Meet agency productivity measures specific to the position.
  • Assume responsibility for a healthy and safe environment.

Requirements:

EDUCATION & EXPERIENCE REQUIREMENTS: RN licensed practice in Maryland.

SCHEDULE: Flexible to meet the needs of the program/clients; ability to work some hours remotely from home; mobile work
