Rehabilitation Assistant - Adult Rehabilitation Program(s) Job at Residential Services-Lanham
General Summary: To provide quality rehabilitation training and support to clients participating in the rehabilitation programs. Responsible for assisting with coordination of care on an as needed basis. Demonstrates excellent interpersonal skills with clients, their families, staff, internal and external staff at all times. Instructs, assists, and monitors clients in accordance with individual rehabilitation plans. Will transport clients and monitor medications as needed.
Responsibilities:
- Orientation Toward Customer Relations: Orients new members, visitors, and volunteers to the program and its offered services.
- Participation in Rehabilitation Planning: Functions as a team member to assist in developing rehabilitation plans that meet client's needs, and by informing team members of significant client issues. Provides rehabilitation counseling to the consumers a part of the program.
- Provision of Clinical Services: Designs and runs designated number of groups/classes. Instructs, assists, and monitors the day-to day program activities and assists clients in meeting program expectations. Focuses on symptom management tasks, independent living and coping skills. Assists in maintaining good consumer relations.
- Client Safety: Assists in providing a safe and therapeutic environment according to established policies and procedures. Provides additional and or relief coverage as indicated by staff shortages and or unexpected or unusual clinical needs of the clients.
- Documentation and Reporting: Completes written documentation of rehabilitation plan, services, behaviors, and progress in compliance with Mosaic Community Services and program standards. Reports pertinent observations to rehabilitation team in a timely manner.
- Participating in Improving Program Effectiveness and Efficiency: Participates in the evaluation of program effectiveness and efficiency and works to ensure continuous improvements quality of services.
- Monitoring Medications: Completes assigned related trainings. Monitors client prescribed medications on-site and off-site as assigned by direct supervisor.
Requires:
- Work requires the ability to read, write, and speak English well enough to maintain documentation and other record keeping tasks and working knowledge of specific patient population served (i.e. children and adolescents or adults with mental disorders) - normally acquired through high school education or higher, including a relatively brief period of special training in high school/college. A.A. degree in Counseling, Mental Health, or Human Services is preferred.
- Work requires one-to-three month orientation period to obtain sufficient familiarity with behavioral rehabilitation methods, procedures and techniques, program policies and procedures.
- Must possess valid driver's license at time of appointment. Must have a current driving record with less than (3) points.
- Must have a vehicle during working hours and utilize that vehicle for work related duties, including transporting clients as necessary.
- Must have interpersonal skills sufficient to establish therapeutic relationships with mentally ill clients and to communicate and work effectively with clients in accordance with rehabilitation plans.
- Must possess analytical skills sufficient to handle routine administrative details, such as completing Program reports.
- Work requires standing and walking, walking up and down stairs to monitor clients and participate in program activities, driving program vehicle to transport clients, and the ability to physically assist clients with daily tasks.
- Work requires basic computer skills for completion of required documentation.
Working Conditions: Normal activity center environment, where there is no physical discomfort due to temperature, dust or noise. Depending on the position may have some exposure to cigarette smoke in area designated for clients' smoking. Works in client care environment where there is some exposure to communicable diseases.
