RN Assessment Coordinator (RNAC) Job at Milton Nursing and Rehab Center
POSITION SUMMARY:
The Registered Nurse Assessment Coordinator (RNAC) is responsible for the accurate and timely completion of all Medicare/Medicaid case-mix documents in order to assure appropriate reimbursement for care and services provided within the Facility. Conducts concurrent Minimum Data Set (MDS) reviews to assure achievement of optimal allowable Resource Utilization Group (RUG) category. Oversees the overall process and tracking of MDS/Prospective Payment System (PPS) documentation and submission. He/she will integrate nursing, dietary, social recreation, restorative, rehabilitation and physician services to ensure appropriate assessment and reimbursement.
EXPECTATIONS:
Availability:
- Full time - 80/hrs per pay period expectation.
- Monday through Friday- day shift hours.
- Occasional weekends as necessary.
Certification:
- Current, active license as Registered Nurse in Pennsylvania.
Education:
- Graduate of accredited school of nursing. Bachelor of Science Degree in Nursing is preferred.
- AANAC Certificate strongly recommended
Skills:
- Knowledge of geriatric and general chronic care nursing principles and professional standards of nursing practice and ability to apply to resident-specific circumstances.
- Able to identify, implement and evaluate appropriate objectives and interventions for residents.
- Demonstrated competency in clinical skills needed for facility resident population.
- Demonstrated competency in physical, behavioral and functional capacity assessment of long term care residents including use of the state-approved Resident Assessment Instrument.
- Current knowledge of laws and regulations that influence provision of care and services in nursing facilities.
- Working knowledge and ability to interpret and demonstrate facility policies, procedures to direct care staff.
- Carries out all duties in accord with the facility mission and philosophy.
- Demonstrates knowledge of and respect for the rights, dignity and individuality of each resident in all interactions. Able to act as role model for unit staff by protecting and promoting residents' rights in all interactions with residents and families.
- Appreciates the importance of maintaining confidentiality of resident and facility information.
Experience:
- One year of clinical experience.
- Experience with MDS completion, reimbursement, clinical resource utilization is highly desirable.
