Hours: 3 days, 24 hours a week, day shifts.
Location: Saints Campus

The Breast Health Patient Navigator, under the supervision of the Director of Imaging Services, performs/supervises services according to the policies, procedures, philosophy, and objectives of the department and hospital. The Navigator works cooperatively within the department and other services to create a system of quality health care. Serves as a liaison to community and Lowell General physicians to facilitate and enhance the quality of care of the patient from the time of diagnosis through treatment and recovery.

The Breast Health Patient Navigator will serve as a knowledgeable individual who will afford patients with convenient access to needed program services, facilities and equipment that may not be readily available or accessible without assistance or intervention on their behalf.

  • Demonstrates use of clinical skills and expertise in the area of breast disease, breast cancer, breast cancer diagnosis and treatment, and breast cancer survival issues
  • Conducts comprehensive history and physical examination including patient’s psychosocial needs and potential barriers to care
  • Provides patient education and support utilizing knowledge of adult learning competencies and education and counseling skills.
  • Utilizes available educational resources and support services to provide comprehensive care
  • Effectively and efficiently plans, coordinates, implements and monitors care across service lines for patients requiring breast health care
  • Serves as a liaison and resource for patients, providers, and staff at LGH and the community for the purpose of providing patient centered, evidenced based quality breast health care
  • Communicates effectively with diverse audiences including professional staff, patients, community, media and other service agencies
  • Works collaboratively with members of the Cancer Care Committee, medical staff and administration to provide quality comprehensive care to patients to and ensure that barriers to treatment are overcome
  • Works collaboratively with colleagues in the Radiology Department, Surgical department, and Cancer Center to promote and provide comprehensive, quality breast health care at Lowell General Hospital
  • Develops and implements professional development and community education outreach programs to advance breast health knowledge and evidence based practices
  • Demonstrates management skills reflecting self-motivation, initiative and fiscal responsibility

Requirements:

  • MA RN license and current BLS
  • Minimum of 2 years Oncology experience required
  • OCN Preferred
