Responsibilities
The Case Management Coordinator I is a clinician who is responsible for ensuring timely and appropriate entry of home infusion referrals into our computer database. This individual will negotiate with payors for clients receiving home infusion therapies. Responsibilities include communications with the clinical staff (RN's and pharmacists) of the insurance authorizations/coverage issues for individual clients. This individual will work with The Home Care Network (THCN) staff to communicate and document the nursing visit authorization and plans of care.

EDUCATIONAL/TRAINING REQUIREMENTS:
RN License, BSN degree. Certification in the field of Case Management a plus.
2-3 years combined experience in Home Infusion Therapy or Case Management environment.
CERTIFICATES, LICENSES, AND REGISTRATION:
RN Licensure in PA
BCLS
Driver's License
EXPERIENCE REQUIREMENTS:
2- 3 years of experience in the home infusion field as a clinician or in the case management arena or a willingness to learn these therapies through intensive orientation. Experience with coordination of care/scheduling and/or supervisory experience are a plus.

ESSENTIAL FUNCTIONS:
- Full Time (40 hours/week; Monday-Friday 830a-5pm in King of Prussia office).
- The Care Team Coordinator (Outpatient coordinator) is primarily responsible for scheduling infusion nursing visits for a large census of patients.
- Responsibilities include phone support and triage of patient calls, interactions with referral sources, infusion nurses, and multiple disciplines in the home infusion and medical care teams.
- The outpatient coordinator serves as a clinical and information resource for home infusion nurses and other staff regarding the plan of care for patients receiving infusion therapies at home
- Requires the ability to receive, assimilate and relay large amounts of information in a high sensory environment.
- Organization and ability to multi-task are key.
