About the Project:

The Division of Behavioral Health and Recovery (DBHR) received funds from the Substance Abuse Mental Health Services Administration (SAMHSA) Targeted State Response Grant to develop six Hub and Spoke (H&S) projects in six areas of the state. Funding for this project is part of a commitment from the 21st Century Cures Act to address the Opioid Epidemic. The H&S model creates a coordinated, systemic response to the complex issues of opioid addiction among the Medicaid and low-income populations, focusing specifically on medication assisted therapy (MAT) for individuals with opioid dependence. The integrated care model connects a network of community providers around a central hub that offers a MAT component to all patients seeking services for opioid use disorders. The Hub must provide at least one agonist (Methadone, Buprenorphine) and one antagonist medication (Naltrexone, oral and injectable).

Supervises: Spoke Care Navigators

Primary Responsibilities:

· Provide medical support to the prescribing physician and other waivered prescribers.

· NCM will be in charge of:

o Patient screening

o MOUD education

o Assisting with MOUD inductions

o Taking vital signs and conduct EKG

o Drug testing

o Lab work

o Medical assessments

o Charting

o Care planning including stabilization, maintenance, and ongoing coordination of follow up care

o Relapse prevention

o Support for patient self-management

o Observation of the patient

· Perform other duties as assigned by the Medical Director.

License, Education, Experience:

· Registered Nurse (RN) license in Washington State. (BSN required)

· Minimum 1-year experience working with individuals with issues of addiction and Clinical knowledge of Hep-C(Preferred)
