RN/LPN Job at Azura Memory Care
We are seeking an RN/LPN to oversee our Monroe Community. The Registered Nurse is responsible for providing professional and quality person centered care for residents with Alzheimer’s disease, other types of dementia and residents with other medical or nursing needs to promote their highest well-being.
RN/LPN Responsibilities
- Practice a holistic approach to meet residents’ needs and to promote their highest well-being.
- Assess residents prior to admission, when able.
- Visit and reassess current residents who may be in acute care or another long term care setting.
- Utilize nursing process by demonstrating appropriate assessment, planning, implementation, and evaluation of the resident’s care, as evidenced by staff observation.
- Conducts state approved Medication, Standard Precautions, and First Aid Training, once an approved trainer, as needed.
- Provides education and training to team members monthly or as needed to ensure best clinical practice is being performed.
- Provides guidance to Executive Director with clinical processes.
- Document accurate and ongoing assessment of residents’ status. Document resident care which reflects nursing intervention, resident response to care provided, patient needs, problems, capabilities, limitations, and progress toward goals. Documentation may include evidence of appropriate resident/family teaching and the understanding of these instructions in the medical record.
