Qualifications:

• Bachelor’s Degree in Physical Therapy
• NYS License as an Physical Therapist

Job Responsibilities:

• Provide diagnostic evaluations and re-evaluations as scheduled and assigned.
• Provide recommended clinical services to children as assigned
• Participate on interdisciplinary treatment teams.
• Make appropriate referrals to other departments or services where indicated.
• Act as a resource to children and their families in regard to clinical programs by encouraging their participation in the therapeutic process through shared decision-making and training.
• Participate in the screening evaluation process of new referrals for the CLC.
• Train school personnel in the therapeutic carry-over activities and safety practices related to the therapeutic process.
• Prepare and maintain accurate reports within the appropriate time frame.
• Participate in CLC/Agency Committees.

Physical Demands:

• Transport students in their wheelchairs, school chairs, walkers, standers, etc.
• Aide in the transfer of students from various equipment (as needed).
