Position Purpose
The Supports Intensity Scale (SIS) assessor completes SIS assessments for people who receive services in intellectual and developmental disabilities (IDD) waivers through Home and Community Based Services (HCBS). This position will work 20 or less hours per week. The SIS is a standardized assessment tool designed to measure the pattern and intensity of supports that a person needs to be successful in community settings. The State of Colorado uses this assessment to help determine support level of needs that impact supports and funding for each individual person. Assessments are completed as people are enrolling into services and when requested and approved by the State of Colorado.
Essential Duties
- Provides schedule of available times to complete assessments.
- Facilitates meetings to complete the SIS assessment.
- Accurately scores the SIS assessment.
- Enters all data into the state prescribed system.
- Communicates with team members and RMHS about test scores, meetings, and any issues or concerns while completing the SIS assessment.
Knowledge, Skills and Abilities
- Trained and certified to complete SIS assessments.
- Communicates effectively with teams.
- Experience and knowledge of strong customer service practices and person-centered approaches.
- Knowledge of issues related to people with disabilities, long term services and supports, family dynamics, and community resources.

- Trained and certified to complete SIS assessments.
Preferred Qualifications
- Experience completing SIS assessments.
