Generations at Elmwood Park in Elmwood Park, IL, has an immediate opening for a Full Time Social Services Coordinator. The hours are Monday-Friday, 8:30 am - 5:00 pm, with Occasional Weekends.
*The Pay Rate is $17.00 per hour, Plus Full Benefits*

Responsibilities of the Social Services Coordinator:
- Assist the Social Services Director with planning and implementing social services programs
- Assist with respiratory unit residents
- Greet new residents
- Help acquaint residents with community resources
- Check in with residents regularly to ensure satisfaction, follow up on behavior reports
- Complete periodic assessments
- Help with discharge planning (order equipment, arrange home health, etc.)
- Attend care plan meetings with families, build rapport
- Assist with resolving grievances
- Assist Social Services Director as needed

Requirements:of the Social Services Coordinator:
- At least one year of related experience required
- Some College desirable
- Excellent relationship building and communication skills
- Empathy for and understanding of older adults
