Social Work Case Manager - Home & Community Care Job at Scott County, MN

Provides case management for home and community based service programs for Scott County residents who are utilizing a CAC, CADI, EW, AC, or BI waiver. Performs highly responsible and professional social work case management, involving a wide variety of responsibilities.
**Note: Salary range listed is the 2022 hiring range.**
Provides case management and direct services for waiver participants in a trusting and therapeutic manner, performing functions required by federal and state laws and rules.
Works in partnership with individuals or families to develop individualized service plans based on MnCHOICES assessments; coordinates and directs service to ensure implementation of plans.
Performs ongoing case management to include field visits, facilitating meetings, identifying progress made toward goals, documenting files and notes, ensuring quality of care, etc.
Ensures the safety of clients through the development of community service and support plans, safety plans, and back-up plans; assists in times of crisis to influence positive outcomes.
Collaborates, plans, implements, evaluates and consults with other professionals and organizations to provide for and promote a high quality of life for home and community care populations
Drives a vehicle to transport self or clients to activities or appointments as the needs of service dictate.
Provides education, skill development, problem-solving and support to clients and their families to empower and enable them to live independently; advocates for clients waiver participants to maintain quality services and programs.
Flexibility in scheduling may be required to meet the needs of the clients.
Performs professional level case management establishing trusting, helpful, and empowering relationships with clients to assist individuals in success w/decision-making and self-determination.
Maintains working knowledge of federal and state laws and regulations and performs work accordingly.
Performs other related duties as required or assigned.
Requires a Bachelor's Degree in a Human Service related field and two years of professional social work experience working directly with clients. Strong preference for experience in Home and Community Care waiver service delivery. Master's Degree and bilingual abilities are desirable. Must possess a valid driver's license and provide a reliable means of transportation for the performance of work responsibilities.
Note - Intermittent, Temporary, and PT positions less than 20 hrs per week are not eligible for benefits.
