Social Worker Responsibilities:
- Intake Assessment (Check Eligibility, Complete intake form, provide tour, follow up as needed.)
- Conduct initial assessments
- Admissions Package (Rights, Responsibilities, Emergency, Policy and Procedures, Photo Video authorization, Food Program contract form, Medi-Cal card image for food program.
- Participant agreement form and discuss with ptp and MDT scheduled days of attendance.
- Conduct Initial Home Evaluations and home visits as needed.
- Conduct bi-annual reassessments.
- Individual and group counseling to elderly participants
- Provide various care management services to participants in order to obtain needed government provided services such as IHSS and Access Paratransit
- Provide case management, which might include answering or making phone calls on behalf of participants and/or family, assist with PCP and other appointments, identifying and resolving immediate needs and problems, linking clients to community resources
- Documentation in progress notes of provided case management services. File in ptp chart if relevant for future references.
- Complete reassessments, progress notes, flow sheets, quarterly in a timely manner.
- Assist with updating participants charts with Intake, Admission and Social Work section tab.
- Planning and coordination of discharges and manage discharge folders.
- Development and implementation of Individual Plan of Care.
- Handle Complex cases and/or LCSW cases.
- Work with a multi-disciplinary team to ensure standard of care for participants
- Provide guidance and mentorship to Social Work Assistants.
- Bilingual (spoken and/or written) in Chinese (Mandarin and/or Cantonese)
Job Type: Full-time
Pay: $20.00 - $25.00 per hour

Education:
- Master's (Preferred)
Experience:
- Clinical Social Workers: 1 year (Preferred)
Language:
- Mandarin or Cantonese (Preferred)
License/Certification:
- Clinical Social Worker (Preferred)
