We are currently searching for dedicated Social Workers (Enrollment Brokers) to join our team. Enrollment Brokers conduct intake visits for Home and Community Based Services waiver applicants, and acts as a facilitator for the enrollment process. This role will travel counties as needed working primarily in applicants' places of residence. This position is located in Dauphin County.

POSITION REPORTS TO: Field Supervisor (Supervisor-Enroll/Elig)
EDUCATION/EXPERIENCE: Bachelor's degree from an accredited college or university required; preferably in Social Work or Human Services Field. A minimum of 12 college credit hours in health and human services fields and 1 year of applicable experience may be accepted as alternative to a Bachelor’s degree. Excellent organizational, written and verbal communication skills; ability to interact courteously and effectively with a variety of audiences, including the disability community; ability to handle pressure and perform multiple tasks; ability to work independently; experience entering data into automated information systems; knowledge of local health care networks and providers; ability to maintain cultural sensitivity. Preferred qualifications include three years of human services or health care experience, experience managing staff that conduct home visits, knowledge of Pennsylvania Home and Community-Based waiver services (HCBS), and bilingual capabilities, particularly in American Sign Language. Must be willing to travel to applicants' places of residence throughout assigned region. Occasional evening and weekend hours required. Employment is contingent upon successful clearance of criminal and child abuse background checks and an absence of affiliations with Office of Long-Term Living (OLTL) providers.

JOB RESPONSIBILITIES:
1. Travels throughout assigned county, working primarily in applicants' places of residence
2. Conducts intake visits for waiver applicants and acts as a facilitator for the eligibility determination process, providing assistance from the time individuals are identified as potential participants in an HCBS program to the time they are enrolled as participants or are terminated from the intake process
3. Presents and discusses the concept of the waiver programs, and advises applicants of their rights and responsibilities
4. Assists with application submission, including data entry of application information into HCSIS
5. Assists applicants with investigating Medicaid eligibility as needed
6. Assists applicants with selecting Supports Coordinators that best meet their needs, ensuring consumer control is maintained throughout the application process
7. Consults with the Enrollment Nurse for more difficult cases, as needed
8. Initiates the development of the Individual Service Plan and obtains Department approval
9. Assists with developing relationships with community-based organizations, advocates and stakeholders to gather feedback on improving the application process and removing barriers to enrolling in HCBS waiver programs
10. Fosters and maintains relationships with parties serving our mutual consumers, including physicians, County Assistance Offices, providers, nursing homes and rehabilitation facilities
11. Facilitates the efficiency of the application process, ensuring any stalled cases receive extra attention and support
12. Attends regularly scheduled meetings at central Harrisburg office
13. Reports on schedule availability at regular intervals
14. Performs other duties as assigned by management
