We are seeking a highly motivated and compassionate Per Diem Social Worker to work with our StayingWell department client caseload. As a member of our team, you will provide comprehensive social work services to seniors and their families within their homes. You will collaborate with interdisciplinary team members to develop care plans that promote optimal health, well-being, and independence for our clients.

Responsibilities:

• Conduct psychosocial assessments to identify clients' strengths, needs, and preferences.
• Develop care plans that address clients' physical, emotional, social, and environmental needs.
• Provide counseling, education, and support to clients and their families.
• Assist clients in accessing community resources and services.
• Advocate for clients' rights and preferences.
• Identification of patient and family needs for other home health services and referral for same when appropriate.
• Coordinate medical services.
• Provide caregiver stress relief.
• Participation in development of the total interdisciplinary patient care plan.
• Maintain accurate and timely documentation of all client interactions and interventions.
• Stay current with best practices in geriatric social work and related fields.
• Knowledgeable and compliant with all agency and regulatory procedures including but not limited to Corporate Compliance, HIPAA and DOH.
• Other duties as assigned.

Job Qualifications:

• Master's degree in social work from a school accredited by the Council on Social Work Education.
• Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of Connecticut
• • Minimum of two years of experience working with geriatric clients
• • Certification in care management through NACCM or ability to ability to obtain within 6 months of employment
• Knowledge of and experience with issues related to aging, caregiving, chronic illness, and end-of-life care
• Acceptance of and adaptability to different social, racial, cultural, and religious modes.
• Is able to drive and travel in all types of weather and environmental conditions.

This is a per diem position, with hours and days varying based on department needs. The successful candidate will be flexible, reliable, and able to work independently. Compensation will be commensurate with experience. This position will have the potential to become full time as caseload expands.
