Description

UPA is seeking qualified candidates for The Pat Summitt Clinic Social Worker (PSC) position.

The role of the PSC Social Worker is to identifies psychosocial priorities and needs of patients and families at The Pat Summitt Clinic. Offers support and counseling. Serves as advocate for patients/families with other agencies and links them with appropriate community/medical resources. Provides social services support for patients and families of The Pat Summitt Clinic.

Essential Functions include, but are not limited to:
Links patient, family and caregivers with community/medical resources:
  • Maintains community resources listing to assist patient and caregiver in accessing needed resources. Provides information to patients and families as requested.
  • At direction of providers, sends and communicates referrals for physical therapy, speech therapy, occupational therapy, home health and hospice services.
  • Completes any necessary documentation for referrals.
  • Establishes and maintains relationships with local, regional and state-wide resources by networking and attending required neurology social work seminars and meetings annually.
  • Provides access to medication assistance resources. Order and re-orders medication as requested by the patient or family.
  • Facilitates Support Groups for The Pat Summitt Clinic patients and families.
  • Arranges and communicates placement in geriatric psychiatric units as requested by providers and families. Ensures all clinical documentation is forwarded to next provider of care.
  • Facilitates caregiver training courses for caregivers.
  • Represent The Pat Summitt Clinic at varies events in the community.

Acts as primary contact with patient for follow-up:
  • Contacts patients following resource referral.
  • Communicates with providers for specific patient/family needs or concerns.
  • Provides prompt response to all inquiries. Communicate provider recommendations to patient/caregiver the same day if possible. If unable to reach provider for non-urgent needs, communicates to patient expected time of delay.

Arranges for patient and family long term needs:
  • Contacts community resources.
  • Assists with referrals to appropriate resource.
  • Informs patient/family of referral information.
  • Meets with community resource representatives as needed.
  • Completes and submits all disability or long-term care forms
  • Arranges and communicates Assisted Living or Nursing Home placement information and submits required documentation
  • Educates patient, family and caregivers about Advanced Directives and assists with conservatorship issues as requested

Ensures all necessary patient documentation is available for providers:
  • Ensures patient record contains documentation of: patient/family identified needs, contacts made on behalf of patient, referrals, interventions and follow-up.
  • Ensures patient confidentiality

Provides patient, family or caregiver education:
  • Educates patients, caregivers, and family about tools for better mental and physical health.
  • Provide education to clinical and non-clinical staff as needed or directed.

Interviews patients, family and caregivers as requested by providers:
  • Interviews patient, family, and caregivers during a visit to The Pat Summitt Clinic.
  • Assists providers in assessing patient and family psychosocial needs.
  • Identifies appropriate resources for patient, family and caregivers.

Marginal Responsibilities:
  • Cooperate with co-workers and management to allow for staffing needs and coverage
  • Assist with other staff duties as needed or assigned by Manager.

Requirements

Education:
  • LCSW
Experience:
  • 3-5 years of experience in social work/care management, preferably in an outpatient/ambulatory setting.
