Social Worker II - Full-time Job at WHS Washington Hospital
The Social Worker II is responsible and accountable for the provision of psychosocial assessment, counseling, intervention, and discharge planning to patients and families. Identifies psychosocial issues related to environmental needs, interpersonal conflict, physical and mental illness, using a variety of treatment modalities including, individual, family and group (clinical) counseling, short term therapy, crisis intervention, and bereavement counseling. Provides educational counseling pertinent to discharge plan; coordinates discharge plans and other continuing care needs; serves as patient and family advocate in adjustment to episodes of acute and chronic illness, hospitalization, and lifestyle changes. Assists/mentor the Social Worker I. At a minimum, must perform established Washington Health System Core Social Worker II Skills & Responsibilities.
Shift: Routinely Monday - Friday 9 a.m. - 5:00 p.m. with rotating weekends and holidays.
