Essential Job Duties:
The Social Services Practitioner coordinates the care and services of patients identified in need for intervention across the continuum of care. The individual acts as liason bewteen patient, family, healthcare team and outside community agencies. Develops, facilities, and executes comprehensive plan of care basedd ongoing assessments. Also, specialized in the coordinator of care for patients with complexpsycho-social issues.
Qualifications
Requirements:
- Medical Social work and/or gerontology desired
- Master’s degree in Social Work required.
- LC SW preferred
- LPS Certification after hire.
- CPI is required within 30 days of hire.
