JOB SUMMARY:

Our Speech Therapist is responsible for assisting with communication and swallowing appropriately. Duties will include assessing patients, diagnosing disorders that can impact swallowing and speaking, and to provide treatment for those patients.

QUALIFICATIONS:

  • Graduate of an ASHA approved master’s level Speech Language Pathology program.
  • Must have current Michigan Speech Therapist License.
  • Have current certificate of clinical competence.
  • Should have a minimum of two years of clinical experience.
  • Must have own reliable transportation.

DUTIES & RESPONSIBILITIES:

  • Supervision: Volunteers, high school job shadowing students, school-based or home health aides.
  • Scheduling of customers for therapy and various evaluations.
  • Preparing necessary reports and documentation.
  • Communication with participating care team members in the development and carrying of patient/client care plans.
  • Requests needed equipment and supplies for the operation of the department.
  • Other tasks as assigned by the Rehabilitation Supervisor.
  • Evaluations and Treatment:
  • To be responsible for evaluation of patients as needed and supervision of other caregivers who will be carrying out treatment objectives.
  • To develop evaluation and treatment schedules to meet the needs of the inpatient, outpatient, and home health care patients.
  • Establish and maintain complete and appropriate documentation of evaluation procedures, progress notes, discharge summaries and transition plans.
  • To practice therapy consistent with the American Speech Language and Hearing Association guidelines/Code of Ethics and rules and regulations of the State of Michigan and Medicare/Medicaid program.
  • To maintain a schedule consistent with the needs of the department, as far as possible accommodating the needs of the patients and physicians, while continuing to provide quality patient care.
  • To confer when necessary, with referral sources regarding current treatment and the need to change orders to provide patients with the best care.
  • Records and Reports:

To be responsible for the preparation and submitting of any departmental reports including documentation of patients and special reports as requested by supervisor. Any other reports or records as deemed necessary.

  • Meetings and Conferences:

Such as department meetings, Home Care team conference as needed, and

quarterly QI meetings.

  • Department Maintenance:

Monitor that department is kept neat and orderly, equipment maintained and

supplies are in clean, operating condition.

  • Continuing Education:

To pursue continuing education courses as needed to keep current with new

techniques and treatments in the field of occupational therapy.

To assist with community education.

  • Planning:

Future improvements in procedures and organization of the department.

New Speech Therapy programs to be implemented by the department.

8. Policies and Procedures:

To suggest new or revised policies and procedures for both department and

hospital-wide practices.

9. CQI: Assist in the department CQI program.

10. Demonstrate appropriateness in meeting objectives in age-specifics.
