The Street Medicine Case Manager or Associate Social Worker is responsible for conducting field-based (ie: at tents, in encampments, at community spaces, at SROs) assessments of clients’ needs, including conducting psychosocial and cognitive assessments; developing and continually updating collaborative client care plans; providing referrals to health and psychosocial service resources and programs; and providing informal, field –based short-term psychosocial counseling to address immediate client barriers and impediments to care, including mental health and substance abuse issues. The position is also responsible for the orientation of new staff, interns, and volunteers, and coordinating services within their program as assigned by their direct supervisor. This position will be active with other SFCHC Programs and participate when possible with SFCHC Committees and Events. Direct client work and interaction will be done in the field (tent-side, encampments and in community spaces) and at the office. ASW candidates will also be required to offer street-based therapy to Street Medicine Patients on an ongoing basis as needed.
Essential Functions:

  • Conduct intake, assessment, reassessment, and discharge planning of Street Medicine clients
  • Coordinate with multi-disciplinary care team and support clients with adhering to treatment goals established with their medical providers
  • Provide service referral, linkage, coordination and advocacy services for clients
  • Conduct quality assurance on referrals to ensure that follow-up and referral completion is reached, if needed
  • Provide case management services following standards of practice
  • Develop and renew clients’ individualized case management treatment plans
  • Orientation for new interns, volunteers, and new staff
  • Conduct individual counseling and develop service goals with clients
  • Provide information and referrals to external services and monitor referral completion
  • Benefits advocacy and navigation when needed (i.e. ADAP enrollment, Medi-cal/Medicare linkage)
  • Complete necessary documentation, Units-of- Service (UOS) reports and treatment plans in a timely manner
  • Participate in case conferences, monthly staff meetings, and in-service training
  • Provide crisis prevention, intervention and safety planning services.
  • Participate in SFCHC groups and committees
  • Navigate and outreach as needed
  • Perform other duties and tasks as assigned
  • Provide ongoing tent-side therapy (as appropriate based on education and training, ie: ASW applicants)

Minimum Qualifications:

  • Fully vaccinated against COVID-19, with booster, and able to show proof of vaccination
  • BA in a health sciences or social work-related field or AAA/CMH/CHW OR comparable work experience
  • Two years of experience providing counseling, advocacy, education and service coordination and/or outreach work to disenfranchised communities.
  • Knowledge of HIV/AIDS, STIs, substance use, mental health, social service and probation system
  • Familiarity with issues in communities of color.
  • Knowledge of harm reduction philosophy, client centered counseling and motivational interviewing techniques and trauma informed care
  • Experience working with diverse sexual/gender orientations, unhoused people, limited English speakers, and/or multiple-diagnosed individuals
  • Familiarity and experience with Microsoft Word and Outlook programs
  • Must be adaptive, flexible and a team player
  • Strong verbal and written proficiency in English
  • Clinical Reviews and housing assessments
  • Detail-oriented with strong follow-through skills, including attention to and respect for detail
  • Demonstrated excellence in attendance and reliability, interpersonal, organizational, verbal, and written communication skills
  • Must be able to sit, stand, and walk for extended periods of time
  • Up-to-date with vaccinations, especially seasonal flu shot vaccine
  • Proof of TB clearance (issued within the past 12 months or less)

Preferred Qualifications:

  • ASW trained
  • Experience with Epic, Carelink or other EMRs
  • Spanish speaking
