Employment Type: 1.0 FTE non-exempt

Starting Salary: $25/hour or depending on experience

About UpValley Family Centers (UVFC): Our mission is to provide guidance, support, and resources in the community, in the home and for the individual, so that everyone can achieve a better life. We reach all ages with a variety of programs and serve as essential resource hubs for residents living in the upper valley of Napa County. We maintain partnerships with over 40 early childhood programs, school districts, nonprofits, city and county agencies.

The Student and Family Case Manager provides case management and wellness services to students and families, and is an integral member of UVFC’s Student Assistance Program. UVFC uses case management as a means for achieving client wellness through assessment of needs and strengths, identification of resources, education, communication, and advocacy to accomplish client-directed goals. He/she works collaboratively with UVFC staff and partner organizations to support students and families during times of crisis and helps people to reduce and manage stress. This position reports to UVFC’s Family & Senior Services Program Manager.

RESPONSIBILITIES

Case Management

  • Provide direct case management services using a strengths-based approach, over a period of several months through regular check-ins, to individuals facing multiple challenges in their lives. Activities can include conducting a needs and strengths assessment, goal-setting, connection to resources, education and skill-building, and follow-up.
  • Maintain familiarity with community resources and social support networks. Provide clients with information about locally available services, screen them for eligibility and enroll them in appropriate programs. Provide support to clients facing literacy or technology barriers.
  • Assist clients with information and applications for public benefits, including Medi-Cal, CalFresh; refer clients to other internal UVFC programs/services as needed (education, community engagement, economic success).
  • Refer clients to appropriate mental health services and track waiting lists. Obtain client or parent consent to release information. Keep all records of mental health referrals.
  • Maintain detailed records of all contacts with individuals and families requesting assistance. Maintain organized client data files with appropriate documentation.
  • Regularly maintain/update Apricot client database using good data hygiene practices.
  • Monitor gaps and trends in serving local families. Participate in program discussions using program data to reflect on and strengthen services.
  • Coordinate with partners and promote the student and family strengthening services offered by UVFC, including emergency preparedness/disaster response; domestic violence; youth services; parenting programs and mental health supports.

Student Assistance Program

  • Provide individual support to students and/or their families who face complex, nonacademic challenges, working with school personnel and UVFC’s Education Team.
  • Work with mental health providers to ensure that parent consent forms are signed for students to receive treatment services.
  • Conduct needs assessments; set goals; maintain weekly check ins and monitor process. Update and maintain client progress and program completion in client database system.
  • Regularly participate in multidisciplinary core team meetings in Calistoga school district.
  • When appropriate, make referrals to partner agencies to ensure students obtain nonacademic support so that they can focus better in school.
  • Connect and provide parents with supportive services, including but not limited to support groups, Positive Parenting Program, and other resources to help their children.
Emergency Assistance

  • Respond to clients experiencing a crisis, including but not limited to domestic violence, mental health, or financial issues.
  • Assess client financial needs and goals; complete monthly budgets with clients; complete emergency assistance applications or make referrals to appropriate services; connect clients to UVFC’s financial coaching services or provide them directly, if trained.
  • In times of disaster, assess client needs and provide relief services offered by UVFC or make referrals to partner organizations.
  • Track emergency services provided on client database system.
Other

  • Attend staff meetings, trainings, and other functions as requested.
  • Provide client impact stories or other program data information for program reports.
  • Perform other duties as assigned.
QUALIFICATIONS: An appropriate combination of education and experience to perform the essential duties is required. The ideal candidate will have:

  • BA/BS Degree in social work, sociology, human services or related field is preferred
  • written and verbal communication skills in English and Spanish is required
  • excellent customer service skills and service-oriented
  • experience with and sensitivity to individuals and families in crisis
  • ability to maintain client confidentiality and professional boundaries
  • ability to provide care that is safe, timely, effective, equitable, and client-centered
  • ability to work with people of diverse ages and backgrounds
  • knowledge of Napa County and safety net resources available to students and families
  • ability to work well with staff at all levels of the organization
  • knowledge of word processing, spreadsheet, database management and internet navigation
Preferred skills: (UVFC will facilitate acquisition if needed) case management, client charting, database management, domestic violence training, Positive Parenting Program accreditation.
