Mental Health Association of Nassau County (MHANC) is a not-for-profit membership organization dedicated to improving mental health in our community through advocacy, education, program development, and the delivery of direct services. Our organization is looking for a motivated individual with a calling to help adults with mental illness by becoming a Supportive Housing Counselor.

The Supportive Housing Counselor is dedicated to promoting the well-being of our consumers by providing supportive services to individuals in the Supportive Housing Program that are consistent with their personal recovery goals, cultural background, and beliefs.

Essential Duties and Responsibilities

  • Assist in the admissions and discharges of participants in the program, including assisting the residents in packing/unpacking belongings and processing paperwork required for entitlements, etc.
  • Conduct face-to-face or telehealth contact with participants a minimum of once a month or more as needed
  • Coordinate care if participants are hospitalized including but not limited to; hospital visits, consultation with hospital staff, treatment teams, and family members, coordination of discharge and post-discharge after-care services, and provide updates to the supervisor
  • Complete reports as required, using Electronic Medical Records
  • Assist residents with gathering/completing financial documentation for the purpose of recertifications, leases, etc. in a timely manner
  • Serve as a liaison between participants, landlords, and various treatment team members
  • Provide supportive services include but are not limited to daily living skills, assessment for and linkage to other community services as well as teaching participants how to be appropriate tenants in an apartment setting
  • Conduct medication reviews quarterly or as needed
  • Attend meetings, seminars, and in-service training as required
  • Other duties as assigned
Requirements:


Education
Bachelor’s Degree in a Human Service field Required


Experiences

  • Minimum of two (2 ) years working with the behavioral health population
  • Must have working knowledge of Microsoft Suite
  • Knowledge of electronic recordkeeping a plus
  • Must have basic internet and computer skills
  • Strong written and oral communication skills
  • Must have the ability to multi-task
  • Crisis management skills are a plus

Driving “clean” NY State driver’s license, required and must have own transportation. Travel to various MHANC sites within Nassau County
