Note - This position may include service delivery as needed in the outlying counties within Circuit 2 to include occasional case assignments. Experience in rural areas with case management and service delivery is a plus.
POSITION SUMMARY: This position is responsible for the overall coordination and facilitation of the delivery of direct services to youth admitted to Project Connect in a designated area or circuit. The Transition Specialist provides guidance to the youth in strengthening and appreciating his/her support system, as well as assisting members of that support system in providing healthy and helpful assistance to the youth. The Transition Specialist is supervised by the Area Director and Lead Transition Specialist (if applicable) with whom they will provide reports on a weekly basis.
POSITION REQUIREMENTS:
Bachelor’s Degree with 2 years of experience working with at risk youth. Experience in the following: community partnership coordination, service plan development, collaboration with various agencies, resource development, case management, and effective utilization.
POSITION RESPONSIBILITIES: Includes: writing detailed case notes, writing Monthly and Discharge Reports for stakeholders, service needs assessment, comparing and validating information gathered by common sources (youth, family, residential program staff, courts, school district), Development of Individual Service Plan, referrals for service in coordination with stakholders providing updates to the completion of the ISP, discussing barriers to goal completion and referrals to services while leading a Community Action Team, recruiting mentors for youth, recruiting individuals to form a team of support from both internal and external sources..
