New Glarus Home invites individuals with an ASCM or ACE Certification in exercise physiology and or certified personal training certification to apply for our position acting as a Community Wellness Coordinator.

This position is responsible for strategically growing the wellness services of the New Glarus Home. This position is specifically responsible for representing New Glarus Home on Various levels while cultivating relationships, maintaining and growing a client base.

Responsibilities include assisting in designing, developing, implementing and sustaining a Wellness Program to suit the needs of New Glarus and surrounding communities.

This position will assist in creating community initiatives, which allow the facility to raise its public exposure. The ideal candidate will perform wellness presentations, teach classes, and be driven to meet goals of members on campus and in the community. Other duties are as follows:

• Works directly to market programs and services and enroll clients for participation in Wellness initiatives
• Performs face-to-face social-related assessments for members upon enrollment.
• Assists in developing relationships with professional referral sources and introduces wellness services to the public. (i.e. physicians, hospitals, senior centers, SNF's, etc.)
• Maintains close communication with community nursing staff and activities staff to ensure an optimal plan of care for each wellness client
• Actively works to increase and maintain wellness caseload goals. Converts appropriate prospects into clients
• Keeps detailed documentation on each wellness client using evidence based functional outcome measurements
• Maintains tracking systems and software associated with Wellness department on days they are the lead for the Wellness Center

This position is directly responsible tto the Director of Wellness and Life Enrichment
