Womens Health Services Coordinator Job at Nuvance Health
Nuvance Health has a network of convenient hospital and outpatient locations — Danbury Hospital, New Milford Hospital, Norwalk Hospital and Sharon Hospital in Connecticut, and Northern Dutchess Hospital, Putnam Hospital Center and Vassar Brothers Medical Center in New York — plus multiple primary and specialty care physician practices locations, including The Heart Center, a leading provider of cardiology care, and two urgent care offices. Summary:
The Women’s Health Services Coordinator will partner with key Nuvance Health and community-based stakeholders to develop, maintain, and enhance programs designed to serve women’s health in the greater Sharon Hospital region. The position reports directly to the Chief Nursing Officer of Sharon Hospital. Programs will include preventive, acute and chronic health care needs of women across the lifespan continuum.
Responsibilities:
1.Partners with community agencies, Nuvance Health care providers, community-based providers, schools and other stakeholders to develop women’s health care programs and services in the greater Sharon Hospital region.
2.Develops a wide range of wellness and prevention programs to address the varying needs of women across the age continuum.
3.Designs and supports women’s education programs and forums.
4.Develops access strategies for women with active or chronic healthcare needs within Nuvance Health system.
5.Assesses need for and develops psychosocial support needs of women across the age continuum.
6.Acts as liaison between providers, the care team, and the patient to ensure adequate patient navigation. Patient navigation responsibilities include but are not limited to identifying patient needs and facilitating care coordination related to financial, cultural, clinical, logistical, and educational services.
7.Partners with Nuvance Marketing to build awareness of women’s services
8.Explores and pursues grants and other funding sources for women’s health programs.
9.Develops a relevant method to evaluate program outcomes.
10.Conducts outreach regarding women’s services in the region.
11.Maintain and Model Nuvance Health Values.
12.Demonstrates regular, reliable and predictable attendance.
13.Performs other duties as required.
