YDHP Diversion Case Manager Job at The Montrose Center
Overview:
The Case Manager provides housing navigation and case management services to youth and young adults (YYA) who are experiencing literal homelessness or are at imminent risk of becoming homeless, with a prioritization and emphasis on YYA who are part of the LGBTQIA+ community. Services provided include navigating potential hosts opportunities and local housing programs, assisting YYA in self resolution strategies, as well as provision of basic needs, referrals, and crisis intervention. This position provides administrative support by maintaining client files, entering data, and completing required documentation in compliance with contracts.
Duties include:

- Provide client-centered, trauma-informed case management services to diverse system involved YYA and help to navigate housing resources and opportunities and coordinate services with other agencies.
- Meet with individual clients to establish a safe, healthy case management relationship based on participation, problem-solving, and mutual respect.
- Analyze client barriers and challenges, as well as strengths.
- Create appropriate and valuable service plans with clients based on individualized goals and needs.
- Maintain case records and documentation, including but not limited to service plans, assessments, progress notes, intake reports, and incident reports.
- Understand specific needs of YYA who identify as LGBTQ+. Effectively, respectfully serve LGBTQ+ YYA.
- Analyze barriers and challenges as they arise and deliver safe, effective, and youth-focused solutions that are in in line with the Montrose Center’s philosophy.
- Maintain strict confidentiality of client information in accordance with the Montrose Center policies.
- Develop and maintain positive working relationships with community contacts, such as referral sources, landlords, and other service providers.
- Collaborate with various community programs and agencies to advocate for client population needs and provide appropriate referrals; Keep apprised of available community resources.
- Discern how and when to provide approved financial assistance to support the client in their housing stability.
- Regularly and effectively assess and manage crisis situations; decide on appropriate response in crisis situations and act in accordance with standards and laws as indicated.
- Track and enter information into multiple databases for intakes and daily client interactions; ensure accuracy of data.
- Practice professional boundaries with clients and partners and maintain positive relationships.
- Communicate with members of the community, colleagues, and clients effectively and efficiently.Assist YYA who are experiencing homelessness or at imminent risk of experiencing homelessness to navigate resources and connect to immediate housing solutions through creative interventions, community housing opportunities, and support networks.
- Prepare for and actively participate in supervision meetings and all other required meetings or trainings
Requirements:
Requires a Bachelor’s Degree in social or behavior sciences field (or similar)
- Requires a minimum 1 year experience in Case Management, preferably with youth 18-24
- Technological proficiency
- Adaptable time management capabilities
- Effective communication skills Must have flexible hours and reliable transportation.
