The YHSN will be charged with identifying unacompanied and transition age youth who are at-risk or experiencing homlessness and guiding these youth from the street,school,foster care,youth shelters, and shelters,adult shelters, incarceration and mental health system from their current placements toward the Bos COC Coordinated Entry System and ultimately permenant housing.The YHSN will work collabortaively wiht the Housing Director and Chair of the WV Bos CoC Youth Action Board to maintain and enhance relationships with youth providers and housing partners, at both the state and local levels.The YHSN will be tasked with local coordination of the designated DHHR county office(s) and Housing Authorities.
Job Duties:
- Outreach engagement,and coordination of housing resources and supportive services for youth under 24 years old with current or previous foster care and juvinile justice system who are at risk or experiencing homelessness.Seek out and initiate contact with participants, community organizations, government agencies, and faith-based organizations to develop active partnerships with the goal of ending homelessness in Raleigh County and the surrounding areas.
- Seek out and be responsive to training and research the Housing First intiative as it relates to the goal of ending homelessness in Raleigh County and the surrounding areas.
- Complete Client intakes, assess the needs of the youth and young adults ages, verify income and determine participant eligibility, and process applications.
- Organize and maintain participant documentation, files, and dual data entry.
- Ensure program participants meet program eligigibility and documentation meets grant requirements.
- Provide housing location and stabilization services with the goal of acheiving stable housing, permanent connections, education/employment, and promote overall well-being among the youth we serve.
- Maintain Housing list which meet acceptable rental and safety enviroments for youth adults and families.
- Conduct Housing Quality Standard inspections on appropriate housing units and assist participants in obtaining and signing leases.
- Be reponsive to request from participants/guests and program staff.
- Process program related documentation and prepare any required monthly reports.
- Process payments, recertifications, and interm changes and /moves.
- Coordinate special projects.
- Develope strong relationships with community partners which includes, but is not limited to: Continuum of Care Coordinated entry, Youth Action Board, school board: secondary and post secondary, CIS, behavoral health services, Child Protective Services, Law Enforcement, PHA, Local Homeless Shelters, Safe Havens, and other Community Partners.
- Provide Strength Based Case Management Services, develop short and long- range goals based on the need's assesment, and ensures living skills progression for youth.
- Meet with Program participants at a minumum once a week and conduct home visits as deemed appropriate.
- Cleann empty units at participant entry/exit of program.
- Other dutiesa as assigned.
Qualifications:
- Thrives in a fast paced, fun enviroment, and enjoys working with children and families..
- Understand case management trends, best practices. and assessment.
- Demonstrates the ability to work effectively as a member of multidisciplinary team.
- Possess ther ability to communicate with clients and staff efficiently and effectively.
- Expeirence having solutions-based approach to challenges and ability to manage and de-escalate crisis situations.
- Ability to follow directions and seek professional developement.
- Expeirence working with dual data entry.
- Operate basic officce equipment and obtain the skills to utilize the most up to date Microsoft programs.
- Experience working with youth and youth adults, Non Profit Organizations,Department of Health and Human Resources,property management copanies and housing authorities preferred.
- Knowledge of understanding of WV Landlord Tennant Laws, preferred.
- Possess the ability to prioritize task to meet deadlines.
- Must pass a background check, drug screening, and have a High School Deploma or equivalent.
- Must Have a valid WV Drivers license.
Job Type: Full-time
Pay: $10.00 - $15.00 per hour
