POSITION SUMMARY:

Under the supervision of the Community Health Director, the Administrative Assistant will support referral coordination of the YMCA’s Diabetes Prevention Program and daily operations related to the program, scheduling Lifestyle Coaches, while ensuring the program achieves quality performance outcomes.

They are responsible for managing the functions of the program in accordance with the stated mission, goals, and policies established by the administrative staff, governing committees, and Board of Directors of the YMCA.


ESSENTIAL FUNCTIONS:

  • Responding to inquires about the Diabetes Prevention Program via email, HIPAA secure fax or direct message / electronic referral.
  • Screening and enrolling potential participants and following up with referring physicians, if applicable.
  • Entering participant health data in HIPAA secure data platform and membership database.
  • Assisting with retention and commitment of program participants through readiness assessment.
  • Serving as a liaison, ambassador, and advocate for the program within the YMCA and leadership team.
  • Obtain HIPAA certification, annually and following all internal policies to keep patient health records protected.
  • Organize and update rosters for Lifestyles Coaches who will lead the program (in-person and virtual)
  • Support Lifestyle Coaches during staff meetings with Community Health Coordinator, coach check-ins, and program data entry, as needed.
  • Models relationship-building skills in all interactions. Develops and maintains collaborative relationships with community organizations.
  • Maintains regular, clear, and concise communication within area of responsibility.
  • Handling all administrative duties associated with the program including; support of acquisition and /or development of all materials, invoicing, program material purchases and mailing.

QUALIFICATIONS:

  • Minimum of 1-2 years’ experience working in wellness or community health programs that include screening, readiness assessments, and education & prevention programs.
  • Must have a desire to work with members, non-members, and peers in a fast-paced environment.
  • Strong interpersonal skills, including written and oral communication, decision-making and conflict resolution.?????
  • Ability to initiate and work independently while effectively problem solving.
  • Knowledge of basic health, nutrition, and fitness is encouraged, but not required.
  • Attend trainings and meetings as required.
  • Multi-lingual desired but not required.
