STARTING ON JANUARY 9TH!!

• The Care Coordinator will be given a list of referrals from the health insurance company to call and schedule in home appointments for. (members with kidney disease)
• They will be given a script and will basically educate the member on the company program.
• Their goal is to schedule the in home appointment for the member.
• They will be utilizing only 1 system (salesforce) to enter in all their notes and follow through to make sure the appointment is set and everything goes smoothly.

Training is 8-5 for day one and then 8:30-5:30pm after that for two weeks.

Working Shifts After Training

1. 6am-9pm (8-hour shift in this timeframe) with Fri/Sat off (so shift is Sunday-Thursday) Weekend day shift is 8am-5pm shift.

2. 6am-9pm (8-hour shift in this timeframe) with Sun/Mon off (so shift is Tuesday-Saturday). Weekend day shift is 8am-5pm shift.
