JOB SUMMARY*
To make a measurable and positive difference in the lives of older youth and young adults (ages 16-23), who are coping with a mental health or substance abuse diagnosis, as they transition to adulthood. To ensure quality community-based care to young adults, coordinating them to services and support. Care Coordinators work with young adults to design a wellness plan that is intended to help build the skills needed to improve health, promote wellness, attain personal goals and enhance the overall quality of life. Using a strengths-based and individualized approach Care Coordinators help young adults to develop skills for independence, leadership, confidence, and empower them to take ownership of their lives and achieve their goals.

ESSENTIAL DUTIES*
- The average caseload of 14 young adults with mental health diagnoses.
- Facilitate all areas of services for young adults to include, but not limited to: coordinate mental health services (therapy, medication management, wellness management, etc), Life Skill Development, Employment & education supports, Assessment & Evaluations, connect to community resources, peer support, and establish informal supports, etc.
- To maintain regular in-person contact with clients as is defined by the program.
- The Care Coordinator works with the young adult to develop an individualized domain-based plan, based on identified strengths and needs, including a detailed progressive reactive crisis plan.
- Serve as a liaison between young adults and community providers and assure acquisition of services.
- To prepare, complete, and submit required documentation by due dates in Synthesis to include, but not limited to Progress Notes, Domain-Based Plans, Crisis Plans, Services Authorization Requests (SARs), referrals, measurement instruments, etc.
- To monitor client progress through required in-person and phone contacts with each client, so as to meet both client needs, as well as purchaser expectations.
- To monitor and negotiate the delivery of services and agreements with young adults and their teams, to include modification of Domain-Based Plans as needed, in addition to new authorizations for services or discontinuing existing services.
- To establish and maintain good communication, collaboration, and cooperation with all stakeholders (i.e. Wraparound, provider agencies, BHD, etc.).
- To work and communicate honestly and effectively with young adults, families, team members, and other identified supports to develop and implement problem-solving strategies, listen to concerns, provide crisis services in a manner reflective of both Lad Lake and Wraparound.
- To attend all Wraparound required Care Coordinator training, in-services, and professional development activities as directed.
- Able to work flexible hours, including some evenings, depending on the needs of the young adults and program needs.
- To be a team player with a focus on building a strong team environment for young adults
- To be available in-person, and/or by telephone to provide crisis intervention/emergency services and support to young adults.
- To act responsibly when assisting enrollees after-hours during emergency situations.
- To manage program monies responsibly, recognizing budget limits and procedures for spending.
- To enter SARs for client services in a timely manner while meeting accountability standards.

REQUIRED EXPERIENCE*
Education/Training: Associate's Degree in the areas of social work, psychology, sociology, or criminal justice required. Bachelor's or Master’s Degree preferred.
Skills: Excellent verbal and written communication skills. Ability to organize work and manage time efficiently. Able to relate to young adults and set appropriate boundaries. Energetic, outgoing, positive attitude, self-starter, creative, and a team player.
Experience: Some prior experience in working with youth and/or young adults with mental health either as an employee or volunteer is required. Must have a valid Wisconsin Driver’s license, state minimum auto insurance, and must meet Lad Lake, Inc. driving standards.

Pay: $18.27 - $24.04 per hour
