We are looking for a case manager with a 4-year Bachelor’s degree in social work, rehabilitation, psychology, etc. and at least one-year of paid, formal (non-personal caregiving) experience working aging/elderly persons, persons with a physical or intellectual/developmental disability, etc. If you have the ability to work independently, meet deadlines (documentation, reporting, etc.), conduct assessments and provide supportive services to our clients with a professional and caring methodology, we would like to meet you.

Our case managers meet with individuals in their homes, complete strength-based assessments and develop customized care plans focused on client preferences. Our goal is to improve or maintain the clients' quality of life, independence, and overall health and safety. Working in tandem with a nurse cars manager. our staff collaboratively maintains relationships, coordinates all functional and social supports that promote independence and maintain client safety, and creates a plan of care to improve overall well-being is the core of our program.

This is a full-time, predominantly first shift position requiring a valid Wisconsin Driver’s License, access to a vehicle, personal motor vehicle insurance (meeting minimum coverage limit requirements of the Agency), passing a Department of Justice Background Check and Federal Sex Offender Registry Check, and verification of references.

Requirements:
  • Perform care management services according to the policies and procedures of JFS and the Milwaukee County Department of Family Care, and other funding sources, to include:
    • Complete the social worker portion of the MCO comprehensive assessment and provide input to the SP and MCP accordingly.
    • Enter all work performed in MIDAS case notes within three days of contact.
    • Conduct assessments of the client’s psychosocial history and consult with the assigned registered nurse regarding members’ medical conditions, physical limitations, and functional abilities.
    • Monitor member outcomes related to physical environment, mental and emotional health, social supports, community integration, employment, financial issues, spirituality, and end of life issues based on the member’s assessment.
    • Reevaluate the member’s outcomes at appropriate intervals in order to manage on-going psycho-social issues, assess the member’s response to specific interventions, or determine when a change in condition affects the member’s goals in the area of social or emotional well-being.
    • In crisis situations, implement interventions that balance risk and safety in order to successfully resolve the crisis.
    • Take appropriate measures to ensure the safety of members within the boundaries of member autonomy and choice.
    • Provide or arrange for services and supports that best meet the member’s outcomes and routinely monitor the effectiveness of these interventions.
    • Advocate for members with health, mental health, allied health, familial and community based systems.
    • Communicate outcomes to clients and families.
    • Implement plan and conduct reviews at prescribed intervals.
  • Confer with Supervisor(s) on a regular basis and actively participate in the supervisory process.
  • Correct discrepancies and issues of concern upon receiving monthly feedback on chart audits.
  • Complete care plans and other case recordings in a timely and efficient manner.
  • Maintain a minimum 85% productivity or the equivalent in billable time.
  • Complete all assessments, case notes and service authorization follow ups in a timely manner while complying with DFC policies and procedures.
  • Keep all appointments and meetings up-to-date in the Microsoft Outlook calendar.
  • Participate in triage meetings and collaborate with agency staff and other community professionals.
  • Advocate for client(s) and client groups.

General

  • Must have a valid driver’s license, access to reliable vehicle, and automobile insurance coverage throughout employment.
  • Knowledge, understanding, and/or willingness to learn Jewish culture and traditions and those of the Agency population as necessary.
  • Accept other duties and responsibilities as assigned.
  • Candidates must successfully pass a criminal background check with the Wisconsin Department of Justice and a caregiver background check with the Wisconsin Department of Health Services, the Department of Children and Families and the Department Safety and Professional Services.
  • Candidates must pass a Dru Sjodin National Sex Offender and Wisconsin Sex Offender Registry screening.
