A Day in the Life of:

As a member of our team, you will be responsible for providing individualized service management for consumers of SOAR365 Developmental Services, across multiple programs. You will review documentation collected by central intake and secure any additional information necessary to determine consumers' support needs. Responsibilities and Duties:

  • Develops an individualized service plan annually in collaboration with the consumer, caregiver, case manager/support coordinator, and other stakeholders; reviews and modifies plan to reflect changes in consumer need, preferences, and progress.
  • Monitors progress in service plan achievement through the program's daily documentation, on-site observation of services, and dialogue with caregivers and stakeholders; completes and submits quarterly progress reports.
  • Distributes and collects satisfaction surveys from consumers, caregivers, and stakeholders annually.
  • Attends case management/rehab counselor/support coordinator-convened planning meetings; convenes problem-solving meetings as needed.
  • Serves as the primary point of contact for consumers, caregivers, and others on behalf of consumers; advocates for consumers' best interests; interfaces with a wide variety of professionals, programs, and agencies to enhance service system responsiveness to consumers.
  • Maintains comprehensive documentation according to regulatory, reimbursement, and agency requirements.


Requirements and Qualifications:

  • A bachelor's degree in a related health and human services field is preferred. A combination of experience/training may be considered in lieu of the aforementioned degree.
  • Specialized training, certifications, and relevant work experience required including at least two years of which must include working with persons with disabilities or medical concerns and includes experience in service management and related documentation for persons with developmental disabilities or medical concerns required.
  • Knowledge of community resources and human services network.
  • Knowledge of principles and practices of treatment, supports, and programming.
  • Ability to establish working relationships with a variety of consumers and stakeholders; working skill in interviewing and counseling.
  • Knowledge of regulations, policy, procedure, and other requirements applicable to SOAR365-operated programs.
  • Excellent oral and written communication skills; computer literate with the ability to learn new technology.
  • Ability to analyze complex data and to create reports; ability to complete required documentation accurately and timely.
