Case Manager Behavioral Health (Bachelors Required) Job at Rockford Center
BSW or bachelor’s degree in a recognized mental health field
Essential Job Duties:
1. Conduct psychosocial assessments including treatment plans and recovery plans within three days of admission.
2. Make referrals to providers on behalf of patient needs, complete eligibility and enrollment unit applications.
3. Work alongside social workers to complete tasks as necessary for patient needs such as attendance verifications and coordinating care with outside providers.
4. Coordinate discharges by arranging aftercare and completing crisis safety plans with patients.
5. Conduct and attend regular treatment team meetings to provide case management perspective of the patient with the interdisciplinary team by presenting patient’s case and progress towards treatment plan goals, making treatment recommendations, and actively coordinating each case with all members of the treatment team.
6. Report any incidents or suspected incidences of sexual/physical abuse or other issues to community/state agencies and represent the facility in any legal proceedings.
7. Communicate with referral sources and other appropriate parties regarding patients’ progress.
8. Consistently engages and interacts with our patients and their families by using the treatment principles of Trauma Responsive Care, as demonstrated by the following:
- 1. A basic recognition of the characteristic symptoms resulting from exposure to trauma,
- 2. Having an understanding of and sensitivity to the triggers of trauma and trauma re-enactment,
- 3. Incorporating relevant skill sets into daily interactions with patients and families, and
- 4. Participates in continuing education on Trauma Responsive Care and Recovery.
Job Type: Full-time
Pay: $21,552.05 - $79,426.69 per year
