Program Summary
STARR SF is a multi-agency collaborative partnership program designed to meet one of the most critical community care needs in San Francisco – providing additional residential treatment beds, outpatient case management, and wraparound services for adults with co-occurring substance use disorder and mental health needs who have had contact with the criminal justice system. The overall goal is to reduce incarceration and recidivism through strengthening city-wide initiatives focused on jail diversion, recovery, and community reentry for the target population. We provide centralized intake, assessment, and triage at the Community Assessment Center (CASC), enabling individuals who are diverted or discharged from jail to immediately access SUD/MH treatment.

Under the supervision of a Program Director, the Case Manager follows the principles of harm reduction, trauma-informed care, and recovery and wellness in order to engage clients in therapeutic interventions, link them with appropriate social services such as transitional housing, employment, public benefits and other services to support them on their path to re-entry.

Job Description
Under the supervision of a Program Director, the Case Manager follows the principles of harm reduction, trauma-informed care, and recovery and wellness in order to engage clients in therapeutic interventions, link them with appropriate social services such as transitional housing, employment, public benefits and other services to support them on their path to re-entry.
In partnership with Citywide, the Case Manager will engage clients where they are most comfortable in the community. The Case Manager will perform comprehensive client assessments, establish Individualized Intervention Plans that will guide service provision, and develop crisis prevention/ intervention plans. Activities may include crisis intervention, case management, client advocacy, and psycho-education. Case
Managers will adhere to laws regarding confidentiality and reporting requirements, and maintain accurate detailed observation and progress records for electronic data entry. Additional duties may be assigned as needed in order to meet the requirements of the program.

- Works a flexible full-time schedule to accommodate program/client needs. Some evening and weekend hours are required. This is not a 9-5 position in an outpatient clinic.
- Performs intakes and comprehensive assessments with adults who may have psychiatric and dual diagnosis disabilities (substance use disorders).
- Provides case management services, including evaluations, service plan development, assisting clients in accessing housing, financial, medical and other needed services. Coordinates with other service providers.
- Provides individual therapy to stabilize clients and prevent decompensation.
- Provides crisis prevention planning, crisis intervention and support, as clinically appropriate.
- Conducts aggressive community outreach and engagement in order to provide services to clients safely within the community, as needed, and within the clinic, as appropriate.
- Maintains clinical records after each client contact or service, in accordance with mandated standards of Felton Institute and STARR program requirements.
- Contributes to team meetings and case conferences through presentation of cases, discussion of clinical issues and exchange of information and ideas.
- Provides information regarding STARR program to residents of the SF communityOther duties and responsibilities may be assigned as required in order to meet program goals and objectices.
Minimum Qualifications:
- High school Graduate (or GED) required. Bachelor’s degree in a Social Sciences field preferred (Sociology, Social Work or Psychology, Criminal Justice, etc).
- Minimum one-year experience working with Justice Impacted Adults who are experiencing substance use disorders, mental illness, homelessness or related challenges.
- Interested in engaging and connecting with a diverse, high-need community of adults.
- Ability and willingness to provide non-judgmental encouragement to clients is essential.
- Intermediate knowledge and experience working with computers, including word processing and data entry programs.
- Written communication skills in English adequate to draft basic progress reports and meet the documentation requirements of the LEAD program.
- Ability to develop an Individual Intervention Plan, IIP for each client.
- Ability to connect clients to services that relate to crimongenic needs and risk factors.
- Able to effectively organize and present information in group settings.
- Able to work effectively both independently and collaboratively.
- Verifiable clean driving record (Community outreach, such as home visit and health screenings appointments will be required)
- Valid California Driver’s License, insurance, and ability to travel as required to perform duties.
- Bilingual/bicultural encouraged to apply (Spanish/English).
