St. Francis Nursing Home is an 80 bed skilled nursing facility attached to CHI St. Francis Health acute hospital and Primary Care and Walk-in Clinic located in Breckenridge, MN. Resident centered care is provided by health care professionals in a quiet environment.

Responsibilities
The RN Resident Care Charge will manage, supervise, and coordinate the care of residents on their neighborhood on the PM shift by performing the following duties.
Key Responsibilities
- Evaluates activities of the neighborhood to ensure patient care, staff relations, and efficiency of service.
- Evaluates to ensure standards of care and Federal/State regulations are followed.
- Coordinate resident care with interdisciplinary team.
- Serve as a resource for staff.
- Supervise PM shift staff.
- Review and update care plans as needed.
- Complete MDS assessments as assigned.
- Other duties as assigned.
