The OASIS Lead Clinical Specialist is responsible for OASIS initiatives with primary responsibilities that include acting as a liaison between outsource agency, finalization of OASIS and Coding as needed, assuming responsibility for clinician OASIS assessment, action plan development, training and evaluation to assure on-going improvement in all outcome measures as collected in the OASIS Assessment tool and management of the workflow process through HCHB.
Responsibilities:
- Performs Plan of Care (485), OASIS and chart auditing and tracking as needed.
- Performs oversight and audits on outsource records for compliance and accuracy.
- Provides feedback to clinicians for improvement, consistency and compliance. Maintains records of staff documentation. Supports staff with OASIS implementation and on-going needs.
- Manage workflow through HCHB.
- Coordinates and collaborates with third-party vendor activities related to OASIS workflow
- Conducts educational sessions with staff to improve documentation, distributes changes in regulatory requirements and improve overall record keeping within the Clinical Department. Participates in new employee orientation with 1:1 training for new hires.
- Utilizing principals of OBQI (Outcome-Based Quality Improvement) the OASIS Lead Clinical Specialist tracks Home Health Compare Clinician Scorecards outcomes, provides education and performs outcomes analysis for Value Based Purchasing Compliance.
- Acts as a resource to home health for OASIS related queries and guidance
- Responsible for queries from other departments in relation to OASIS, regulatory guidance and comprehensive clinical oversight as related to F2F, PDGM diagnoses, PDGM billing inquiries.
- In conjunction with OASIS Clinical Specialist participates in Team Huddles as a resource for visit Utilization and PDGM appropriate diagnosis care.
- In conjunction with OASIS Clinical Specialist participates in weekly stand-up meetings with Clinical Home Health Leaders to facilitate OASIS responses from clinicians and partnership with them.
- Responsible for OASIS payor change workflow requests as well as OASIS rejected by CMS
Qualifications:
- Graduation from an accredited School of Nursing
- OASIS Certification (COS-C, HCS-O); ICD 10 certification (HCS-D, BHHC) required.
- Currently licensed to practice as a Registered Nurse in the State of Connecticut.
- A minimum of 5 years of nursing experience; two in a home health OASIS setting.
- A minimum of 2 years homecare experience with a strong knowledge of OASIS, Coding, Medicare and Medicaid homecare regulations.
- Ability to prioritize multiple tasks in an organized manner.
- Ability to develop relationships with outside vendors.
- Computer literate with a functional use of Windows software.
- In-depth knowledge of OASIS requirements and documentations.
