Overview of Position:

The Community Health Worker (CHW) position is a grant-funded position, in which the performance of the CHW will be closely evaluated based on the timely achievement of grant goals, objectives, and outcomes. The objective of the CHW’s role is to improve clinical outcomes and care within at-risk communities in the Diabetes population. This position will provide health education and outreach in a culturally and linguistically appropriate manner. The individual is expected to understand their role on the project, including any goals, objectives, activities, and/or outcomes that fall within their responsibility.

Education and Experience:

  • Two years’ experience in providing patient care or outreach services in a healthcare setting preferred.
  • Bi-lingual in English / Spanish is a required.
  • Must possess a valid driver’s license and be able to frequently travel between HopeHealth sites and other locations as needed.
  • Clean 10-year DMV Report must be submitted.

  • Must be able to manage multiple assignments effectively.

  • None

Essential Job Functions:

  • Establish rapport and relationships with existing and potential patients within the communities served.
  • Convey accurate and relevant information to the individuals while a community setting that compels them and enables them to access and utilize services provided at HopeHealth and partner organizations.
  • Provide basic education to patients in way to promote understanding and self-management of their chronic condition.
  • Empower individuals regarding advocacy on behalf of themselves and their community through outreach and prevention.
  • Utilize a variety of teaching strategies to accommodate various learning styles.
  • Serve as a liaison and advocate between patients, providers, staff, and the Community by facilitating communication and coordinating services.
  • Work closely with the clinical staff to support health education, outreach, patient recruitment, and patient navigation.
  • Appropriately document services and other data in the EMR and provide relevant documentation (as needed) to the Project Lead.
  • Collect and track data to identify trends and report outcomes to demonstrate progression towards grant deliverables.
  • Assist in office administrative tasks, reporting, and duties as needed.
  • Travel to community partners and other locations to ensure community visibility and relationship building.
  • Other duties as assigned.

Physical Requirements:

Must be able to hear individuals on the phone and those who are served in person, and speak clearly in order to communicate information to clients and staff. Must be able to have vision, which is adequate to read memos, a computer screen, registration forms and other clinic documents. Stationary standing, walking, sitting, repetitive use of hands and arms. Lifting/carrying 10 lbs. or more from time to time to move or transport supplies, computer equipment, etc. Must be physically capable of driving and parking the medical mobile unit.
