Do you have a passion for assisting individuals with intellectual and developmental disabilities? AbleLight is looking for a Designated Coordinator to help make a difference in the lives of the People We Support.

AbleLight is committed to helping the People We Support live their best life possible. As a Designated Coordinator, you would be responsible for the coordination of service delivery and evaluation for each person served by the program. This position oversees homes in Hopkins, Victoria, Bloomington and Chaska.

Requirements:
  • Baccalaureate degree in a field related to human services, and one year of full-time work experience providing direct care services to persons with disabilities or persons age 65 or older, -OR-
  • Associate degree in a field related to human services and two years of full-time work experience providing direct care services to persons with disabilities or persons age 65 or older, -OR-
  • A diploma in a field related to human services from an accredited post-secondary institution and three years of full-time work experience providing direct care services to persons with disabilities or persons age 65 or older -OR -
  • A minimum of 50 hours of education and training related to human services and disabilities and four years of full-time work experience providing direct care services to persons with disabilities or persons age 65 or older under the supervision of a staff person who meets one of the previously listed qualification
  • Active Drivers’ License, willingness to obtain CPR / First Aid Certification

Responsibilities
  • Oversee the assigned responsibilities in each person’s coordinated service and support plan and plan addendum
  • Take necessary action to facilitate the accomplishment of outcomes according to requirements of state regulations (MN 245D.07)
  • Instruct and assist direct support staff in implementation of coordinated service and support plan and service outcomes.
  • Partner with Program and / or Area Manager to ensure competency of staff in service delivery activities
  • Evaluate effectiveness of program delivery, methodologies, and progress on person’s outcomes based on measurable and observable criteria for identifying when the desired outcome has been achieved according to requirements of state regulations (MN 245D.07)
  • Provide on-call 24-hour staff assistance during emergencies
