Helpline Youth Counseling (HYC), a nonprofit agency has been providing youth services in Southeast Los Angeles County since 1971 with a primary purpose of providing therapeutic counseling and assistance to at-risk, low income children, youth and their families.
Mission
The mission of HYC is to serve youth and their families by promoting the development of strong individuals and families in their community. This is accomplished through education, therapeutic approaches, advocacy, and the utilization of community resources.

HYC has an immediate opening for a full-time Enhanced Care Management (ECM) Case Manager who will be responsible for providing care to members that promote resilience and support, lifestyle changes, member skill development and self-management.*

Provides multiple strategies for engagement, as appropriate and to the extent possible, including direct communications with the Member. Engagement is offered through in person meetings where the Member lives, seeks care or is accessible by either mail, email, texts and telephone, community and street level outreach.

• Provides an active and progressive approach to outreach and engagement until the Member is engaged.
• Provides culturally and linguistically appropriate communication and information to engage Members.
• Responsible for developing a comprehensive assessment and care plan that is individualized and person centered which coordinates and integrates all of the Member's clinical and non clinical health care related needs.
• Develop strategies to reduce avoidable Member admissions and readmissions and provide adherence support and referral to appropriate services
• Maintain accurate and timely documentation of all job-related activities.
• Coordinate with external care providers and entities providing the member with some level of care
• Coordinate transportation and accompany member to critical appointments as needed.
• Will be required to participate in required trainings and clinical supervision.
• Must have excellent organizational skills and must complete all required reports and documentation in a timely manner.
• Performs other duties as assigned by supervisor.

REQUIREMENTS:*

• Bachelors degree in the behavioral sciences.
• One (1) year's full-time work experience in community-based services field.
• Permanent U.S. residency or citizenship.
• Ability to pass criminal clearance background check.
• Ability to successfully pass a TB screening test.

7. Proof of U.S. citizenship or permanent residence.
8. Willingness to work a flexible schedule, including evenings and on weekends.
9. Current auto insurance, and a good driving record.
10. Valid California state drivers license.
11. Ability to manage multiple tasks, projects and priorities.
12. Excellent administrative and verbal skills.

PREFERRED QUALIFICATIONS:*

• Experience in working within the Southeast Region of Los Angeles County and a familiarity with the population of the area.
• Experience in networking with school and community agencies in providing client care services programs.
• Experience in working in a community-based organization.
