General Responsibilities:

To provide permanency planning and direct emotional support and technical assistance to the families involved in the adolescent’s care and treatment. The Case Planner supports the birth and foster parents’ perseverance with children through challenging periods of extreme and troubling behavior and models appropriate responses and teaching strategies. Guiding adolescent birth parents and foster parents through a proactive process of assessment, goal setting, and treatment planning to address the children’s current emotional/behavioral problems, accomplishes this task. specific treatment plans. Case Planner use the SBC model (Solution Based Casework) as a conceptual framework for working with families, youth and their connections. You make a difference, every day!

Requirements:

BA/BSW, 2 years child welfare related experience, Prior experience working with adolescents and a passionate commitment to permanency for adolescents, Good writing skills, Proven ability to meet deadlines, Ability to work a flexible schedule, including periodic evenings and weekends, ability to travel to community settings throughout NYC

Must possess strong organizational skills, empathy, be comfortable with field work and flexible.

Preferred (but not required)

Bi-lingual (Spanish), drivers license
